6.68.020 <br />issued pursuant to this Section, nor any of <br />the regulations and provisions within this <br />Section or any other local, State or Feder- <br />al regulation. <br />C. No person, other than the one per- <br />mitted, may conduct extrasensory consult- <br />ing; and the permittee shall post the per- <br />mit on the premises in public view to the <br />customer. (Ord. 1770 § 1, 1999) <br />6.68.030 Police Department Permit <br />Application And Processing: <br />A. Every person and their employee or <br />agent who, for pay, conducts, engages in, <br />carries on, or practices extrasensory con- <br />sulting shall file a separate verified Police <br />Department permit application for extra- <br />sensory consulting with the Police Chief. <br />The application shall contain the follow- <br />ing: <br />1. Name, home and business addresses, <br />home and business telephone numbers, <br />and all other information as required on <br />the standard business license form. <br />2. Fingerprints and a recent photograph <br />of the applicant on a form provided by <br />the Pleasanton Police Department. <br />3. A list of services proposed to be <br />provided by the applicant, as well as the <br />fees charged for each service. <br />4. A statement of whether the applicant <br />holds a certificate of ordination as a min- <br />ister, missionary, medium, healer or clair- <br />voyant and whether the extrasensory <br />consulting is to be part of a religious <br />ceremony or service. <br />5. A zoning certificate from the Plan- <br />ning and Community Development De- <br />partment. <br />6. Signed authorization for the release <br />of pertinent records. <br />7. A complete personal history set forth <br />on a questionnaire provided by the Police <br />Chief, and any other information which <br />the Police Chief deems to be reasonably <br />necessary to complete the investigation. <br />8. Agreement to indemnify, hold harm- <br />less the City, its officers, agents, and <br />employees, from and against all claims, <br />losses, costs, damages, and liabilities of <br />any kind, including attorney fees, arising <br />in any manner out of the applicant's <br />extrasensory consulting practices. <br />9. Applicant's agreement that the City <br />shall have the right to enter the place of <br />business named in the permit from time <br />to time during regular business hours to <br />ascertain compliance with permit condi- <br />tions of operation. <br />10. Information relating to any previ- <br />ous revocation of a license or permit in <br />any other jurisdiction including, but not <br />limited to, the date(s) and circumstances <br />of the revocation. <br />11. All convictions within the last five <br />(5) years of the applicant for any of the <br />following felonies or crimes involving: <br />dishonesty; fraud; deceit; moral turpitude; <br />theft; the use of force or violence upon <br />the person of another; the unlawful use, <br />possession, or sale of any dangerous or <br />deadly weapon; and the manufacture, sale, <br />possession or use of a controlled sub- <br />stance as defined by the California Health <br />and Safety Code as it now reads or may <br />be amended. <br />B. Upon receipt of a completed appli- <br />cation, the Police Chief shall process the <br />permit application within thirty (30) days. <br />(Ord. 1770 § 1, 1999) <br />(Pleasanton 5-99) 192-86 <br />
