necessary to provide coverage for eligible dependents; provided, however, in no event <br /> shall the City be obligated to contribute a monthly amount in excess of the monthly <br /> premium for the Kaiser S-1 Health Plan full family coverage. <br /> <br /> Effective November 1,1996 the City's monthly premium contribution for the Kaiser S-1 <br /> medical care benefits shall include the premium for the Kaiser durable medical benefit. <br /> <br /> Effective June 1, 1998, the City shall modify the Kaiser S-1 plan to include the Kaiser <br /> "CH3" chiropractic plan for the employee and eligible dependents. The cost of said plan <br /> shall be bome by the City. <br /> <br /> For the purpose of providing aforesaid health care coverage to eligible regular part-time <br /> employees including job share employees who average between twenty (20) and <br /> twenty-nine (29) hours per week, the City shall contribute an amount not to exceed fifty <br /> percent (50%) of the City's monthly contribution established for full time employees as <br /> set forth in the paragraph above. For regular pan-time employees including job share <br /> employees who average between thirty (30) and thirty-nine (39) hours per week, the City <br /> shall contribute an amount not to exceed seventy-five percent (75%) of the City's monthly <br /> contribution above. <br /> <br />15.2 lteaith and Medical Care - Retirees <br /> <br /> Effective April 30, 1990, for all employees who retire for service after April 30, 1990 the <br /> City shall pay for each year of service, four percent (4%) of the monthly premium for <br /> employee and one dependent of the City's current Kaiser S-1 Health Plan coverage. <br /> Effective April 30, 1990 for all employees who retire for disability after April 30, 1990 <br /> the City shall pay a percentage of the monthly premium for employee and one dependent <br /> of the City's current Kaiser S-1 Health Plan coverage in accordance with the schedule set <br /> forth below: <br /> <br /> y~ars of Service Citv's Contribution <br /> <br /> 10 20% <br /> 11 25% <br /> 12 30% <br /> 13 35% <br /> 14 40% <br /> 15 45% <br /> 16 48% <br /> 17 51% <br /> 18 54% <br /> 19 57% <br /> 20 80% <br /> 21 84% <br /> 22 88% <br /> 23 92% <br /> <br /> <br />
