The term medical disability includes all temporary medical disabilities including <br /> pregnancy, childbirth and associated medical conditions. Total approved paid and unpaid <br /> time away from work because of medical disability prior to and after the birth of a child, <br /> shall not exceed one hundred twenty (120) days. <br /> <br /> Such request for medical leave will be considered only upon provision of a physician's <br /> statement attesting to said disability. <br /> <br /> During extended medical leave, an employee shall not accrue vacation or sick leave <br /> benefits or be eligible for holidays, nor shall such time be considered as service time. <br /> Any employee returning from medical leave shall be required to provide a physician's <br /> release to return to duty. <br /> <br />14.4 ~ <br /> Every employee of the City who is called or required to serve as a juror shall be entitled <br /> to leave from his/her duties with the City during the period of such service or while <br /> necessarily being present in court as a result of such call. The employee shall be paid the <br /> difference between his/her full salary and any payment received by him/her, except travel <br /> pay, for such duty. <br /> <br />14.5 Leaves Resulting from Subpoena <br /> <br /> Leave of absence with pay shall be granted to a person while going to and from court and <br /> answering a subpoena as a witness. The employee shall be paid the difference between <br /> his/her full salary and any payment received by him/her, except travel pay, for such duty. <br /> <br />14.6 <br /> <br /> Military leave shall be granted in accordance with the provisions of the law. All <br /> employees entitled to military leave shall give the Appointing Authority an opportunity <br /> within the limits of military regulations to determine when such leave shall be taken. <br /> <br />14.7 Failure to Return from Leave <br /> <br /> Failure on the part of the employee on leave to report to duty promptly upon its <br /> expiration or within a reasonable time after its expiration shall be cause for discipline. <br /> <br /> Section 15. Health and Welfare <br /> <br />15.1 Hospital and Medical Care - Active Employees <br /> <br /> Effective May 1, 1992, the City shall contribute an amount necessary to provide hospital <br /> and medical care benefits under either the present Blue Cross Plan, Kaiser S-1 Plan or, <br /> Foundation Health Plan I for the individual employee and a portion of the amount <br /> <br /> -17- <br /> <br /> <br />
