24 96% <br /> 25 + years of service 100% <br /> <br /> Upon qualifying for and receiving pans A and B of Medicare, the City shall not be <br /> obligated to contribute a monthly amount in excess of the monthly premium for the <br /> Kaiser S-1 supplement to Medicare health plan for employee and one dependent. In the <br /> case where the eligible employee is deceased, the amount the City is obligated to pay <br /> shall be reduced by one-half(I/2). If the spouse remarries, the obligation by the City <br /> shall terminate. <br /> <br /> The City reserves the fight to meet and confer with the Association for the purposes of <br /> coordinating retired employee's hospital and medical care in the event Federal or State <br /> legislation is implemented that affects retiree's medical insurance. <br /> <br />15.3 Dental <br /> <br /> Effective November 1, 1996, the City shall contribute under the City's Amefitas dental <br /> insurance plan an amount not to exceed eighty three dollars ($83.00) per month to <br /> provide dental care benefits including the current maximum orthodontia coverage of <br /> $1,500.00 for the individual employees and eligible dependents. Effective January 1, <br /> 1997, said maximum contribution shall be increased to eighty nine dollars ($89.00) per <br /> month. Effective January 1, 1998, said maximum contribution shall be increased to <br /> ninety three dollars ($93.00) per month. <br /> <br /> For the purpose of providing aforesaid dental care coverage for eligible regular pan-time <br /> employees including job share employees who average between twenty (20) and <br /> twenty-nine (29) hours per week, the City shall contribute an amount not to exceed fifty <br /> percent (50%) of the city's monthly contribution established for full time employees as set <br /> forth above. For regular pan-time employees including job share employees who average <br /> between thirty (30) and thirty-nine (39) hours per week, the City shall contribute an <br /> amount not to exceed seventy-five percent (75%) of the City's monthly contribution <br /> established for full-time employees as set forth above. <br /> <br />15.4 Uft_hlallralll~t. <br /> <br /> Effective May 1, 1992, the City shall contribute an amount necessary to provide $35,000 <br /> life and $35,000 accidental death and dismemberment insurance coverage for the <br /> individual full-time employee. Effective November 1, 1996, the City shall contribute an <br /> amount necessary to provide $26,250 life and $26,250 accidental death and <br /> dismemberment insurance coverage for the individual regular pan-time employee who <br /> averages between thirty (30) and thirty-me (39) hours per week; and $17,500 life and <br /> $17,500 accidental death and dismemberment insurance coverage for the individual <br /> regular pan-time employee who averages between twenty (20) and twenty-nine (29) <br /> hours per week. <br /> <br /> -19- <br /> <br /> <br />
