Approved College/University Programs: Both the program/degree offered by <br />the college or university in the field of study to be pursued by the <br />employee and the college or university itself must be accredited by the <br />Western Association of Schools and Colleges. External degree programs, <br />external academic programs, and/or correspondence courses shall not be <br />eligible for education incentive pay. <br /> <br />F. Documentation of Educational Achievement <br /> <br />1. Evidence of successful completion of a degree or certificate shall <br /> consist of either of the following: <br /> <br />a. Diploma or certificate indicating the major field of study. <br />b. Letter from the college registrar certifying award of the degree <br /> or certificate and the major field of study. <br /> <br />2. Evidence of completion of the required accredited semester units or <br /> quarter hour equivalents may consist of any of the following: <br /> <br />a. College transcript of grades. <br />b. Official grade slips from each of the courses. <br />c. Letter from the college registrar certifying the completion <br /> of each course with a grade of "C" or better. <br /> <br />Termination of Educational Incentive Pay: Educational Incentive Pay shall <br />be terminated at such time as the employee receiving educational incentive <br />pay is promoted to or otherwise placed in a position requiring the educa- <br />tional level which he currently possesses or a higher educational level. <br /> <br />H. Tuition Reimbursement <br /> <br />1. Only employees enrolled in the Educational Incentive Plan shall be <br /> eligible for tuition reimbursement. <br /> <br />Tuition shall be reimbursed in the amount and at the rate for internal <br />degrees levied by Hayward State University at the time of the employee's <br />enrollment in the courses for which tuition reimbursement is requested. <br />However, the maximum amount of tuition reimbursement shall not exceed <br />$300.00 per year per employee. <br /> <br />3. Tuition reimbursement shall not be received for any individual courses <br /> (or class) not successfully completed with a grade of "C" or better. <br /> <br />Employees submitting requests for tuition reimbursement shall provide <br />the personnel officer an official receipt from the college or univer- <br />Sity verifying the payment of tuition and an official transcript of <br />the grades received for which tuition is requested. <br /> <br />I. Book Reimbursement <br /> <br />Employees enrolled in the Educational Incentive Plan shall be eligible for <br />the reimbursement of the books required for those classes taken as part of <br />their approved educational program. The amount reimbursed shall be the <br />purchase price of the books less the monies received from their resale. No <br />reimbursement for books shall be received for any class not successfully <br />completed with a grade of "C" or better. <br /> <br /> <br />
