12.5 <br /> <br />Maternity Leave <br /> <br />The Appointing Authority may grant, upon written request, <br />maternity leave without pay for up to six (6) months <br />upon determination that such leave will not adversely <br />effect the needs of the City. <br /> <br />section 13. Health and Welfare <br /> <br />13.1 <br /> <br />Hospital and Medical <br /> <br />Effective March 1, 1982, the City shall contribute an <br />amount not to exceed one hundred twenty-five dollars <br />($125.00) per month per employee to provide health care <br />coverage to employees and their eligible dependents under <br />either of the present Blue Cross or Kaiser "S" health <br />insurance plans. Effective June 1, 1982, the City's <br />contribution shall be increased to an amount not to <br />exceed one hundred thirty dollars ($130.00) per month per <br />employee. Effective June 1, 1983, the City's contribution <br />shall be increased to an amount not to exceed one hundred <br />forty dollars ($140.00) per month per employee, except <br />that should the monthly premium for the present Kaiser "S" <br />health plan employee and eligible dependent coverage be <br />increased on June 1, 1983 to an amount in excess of one <br />hundred forty dollars, the City's maximum monthly contri- <br />bution shall be increased on June 1, 1983 by one half of <br />the dollar difference between said monthly Kaiser premium <br />and one hundred forty dollars ($140.00). <br /> <br />For the purpose of providing aforesaid health care <br />coverage to eligible regular part-time employees who <br />average between twenty (20) and twenty-nine (29) hours <br />per week, the City shall contribute an amount not to <br />exceed fifty percent (50%) of the City's monthly contri- <br />bution established for full time employees as set forth <br />in the paragraph above. For regular part-time employees <br />who average between thirty (30) and thirty-nine (39) <br />hours per week, the City shall contribute an amount <br />not to exceed seventy-five percent (75%) of the City's <br />monthly contribution established for full time employees <br />as set forth in the paragraph above. <br /> <br />The City shall contribute for the purposes of providing <br />aforesaid hospital and medical coverage to employees <br />who retire after said date an amount not to exceed <br />Seventy-five Dollars ($75.00) per month for a period not <br />to exceed twelve (12) months from the date the employee <br />retires from the City. <br /> <br />-23- <br /> <br /> <br />
