Section <br />12.1 <br /> <br />12.2 <br /> <br />12.3 <br /> <br />12.4 <br /> <br />In order to receive compensation while absent on sick leave <br />the employee shall notify his/her immediate superior or <br />the Personnel' Officer prior to or within four hours after <br />the time set for beginning his/her daily duties or as may <br />be specified by the department head. When absence is for <br />more than eight (8) hours, the employee may be required <br />to file a physician's certificate or a personal affidavit <br />with the Personnel Officer stating the cause of the absence. <br /> <br />12. Leaves of Absence <br /> <br />Leave of Absence Without Pay <br /> <br />The Appointing Authority may grant a permanent or prOba- <br />tionary employee leave of absence without pay or seniority <br />for not to exceed three (3) months. No such leave shall <br />be granted except in writing and upon written request of <br />the employee setting forth the reason therefor. Upon <br />expiration of a regularly approved leave, the employee <br />shall be reinstated in the position held at the time leave <br />was granted. Failure on the part of an employee on leave <br />to report promptly at its expiration or within a reasonable <br />time after notice to return to duty shall be cause for <br />discipline. Department heads may grant a permanent or <br />probationary employee leave of absence without pay for not <br />to exceed one (1) calendar week. Such leaves shall be <br />reported to the Personnel Officer. <br /> <br />Jury Leave <br /> <br />Every employee of the City who is called or required to <br />serve as a juror shall be entitled to leave from his/her <br />duties with the City during the period of such service or <br />while necessarily being present in court as a result of <br />such call. The employee shall be paid the differenc~ <br />between his/her full salary and any payment received by <br />him/her, except travel pay, for such duty. <br /> <br />Leaves Resulting from Subpoena <br /> <br />Leave of absence with pay shall be granted to a person <br />while going to and from court and answering a subpoena <br />as a witness. The employee shall be paid the difference <br />between his/her full salary and any payment received by <br />him/her, except travel pay, for such duty. <br /> <br />Military Leave <br /> <br />Military leave shall be granted in accordance with the <br />provisions of law. All employees entitled to military <br />leave shall give the Appointing Authority an opportunity <br />within the limits of military regulations to determine <br />when such leave shall be taken. <br /> <br />-22- <br /> <br /> <br />
