ATTACHMENT C <br /> <br /> PROGRESS REPORT <br /> <br />As required in Section 3, Task IV, Item b~ of this Contract, a Progress Report <br />on Leak Detection and Repair shall be submitted by the Grantee by June 1, 1985, <br />to the State Program M~ager. <br /> <br />~ Progress Report will describe the specific efforts and results completed up <br />to May 1, 1985. At a minimum, the Progress Report will include the following: <br /> <br /> a. Copies of all Daily Leak Detection Logs (fully completed), s~mp].e <br /> attached. <br /> <br /> b. Copies of all Leak Reports (fully completed), sample attached. <br /> <br /> c. Copies of all Leak Repair Reports (fully completed), sample attached. <br /> <br /> d. Maps identifying which mains were surveyed, where leaks were located <br /> the rate of water lost (GPM) at each leak, and which leaks were <br /> repaired. <br /> <br /> e. Sunmary tables displaying: <br /> <br /> - Total number of leaks detected by project. <br /> <br /> - Total number of leaks reported during project from other sources. <br /> <br /> - Total estimated rate of water lost (GPM) from leaks discovered in <br /> project. <br /> <br /> = Number of miles of main surveyed for leaks. <br /> <br /> - Number and description of listening points. <br /> <br /> = Number of detected leaks repaired. <br /> <br /> - Total number of person hours expended on leak detection. <br /> <br /> - Total cost of leak detection. <br /> <br /> - Total number of person hours expended on leak repair. <br /> <br /> - Total cost of leak repair. <br /> <br /> - A statemen~ comparing the amount of leak detection and repair <br /> completed with respect to the amount scheduled in the Leak <br /> Detection and Repair P!an (submitted per Section 3, Task I, T_tem 10 <br /> of this Contract). <br /> <br /> - If the work is behind schedule, then include a statement describing <br /> what improvements will be made in the Grantees efforts to bring the <br /> Project on schedule. <br /> <br />28 Contract B-550~'D <br /> <br /> <br />
