14.2 <br /> <br />Dental <br /> <br />Effective March 1, 1984, the City shall contribute an <br />amount not to exceed thirty f~ve dollars and fifteen <br />($35.15) per month per employee to provide dental <br />care benefits under the City's present Banker's Life <br />Nebraska Plan for the individual employer and eligible <br />dependents. <br /> <br />14.3 <br /> <br />Life, Accidental Death & Dismemberment and Long Term <br />Disability <br /> <br />The City shall contribute an amount necessary to provide <br />$10,000 life and $10,000 accidental death and dismember- <br />ment insurance coverage for the individual full time <br />employee. Effective July 1, 1980, the City shall improve <br />the aforesaid coverage to $20,000 life and $20,000 acci- <br />dental death and dismemberment coverage. <br /> <br />Effective March 1, 1984, the city shall contribute <br />an amount necessary to provide long term disability <br />insurance under the present Standard Insurance Company <br />group policy for the individual full time employee; <br />provided, however, that in no event shall the City <br />be obligated to contribute an amount in excess of <br />One and Seven one-hundredths Dollars ($1.07) per one <br />hundred dollars ($100) of monthly salary of the first <br />$2,250 of monthly salary not to exceed a total con- <br />tribution of Twenty-four and Eight One-hundredths <br />Dollars ($24.08) per month per employee. Effective <br />June 2, 1984 said maximum monthly City contribution <br />shall increase to one and twenty five one hundredth <br />dollars ($1.25) per one hundred dollars ($100) of the <br />first $2,250.00 of monthly salary. Should the present <br />monthly premium for said long-term disability insurance <br />of one and seventy nine one hundredth dollars ($1.79) <br />increase, however, the City shall increase its maximum <br />monthly contribution rate by one half (1/2) of said <br />increase up to a maximum additional increase of twenty <br />five cents per one hundred dollars ($100) of the first <br />$2,250 of monthly salary. In no event during the term <br />of this agreement shall the City be obligated to con- <br />tribute an amount in excess of one and fifty one <br />hundredth dollars ($1.50) per one hundred dollars <br />($100) of monthly salary of the first $2,250 of <br />monthly salary not to exceed a total contribution of <br />thirty-three and. seventy-five one hundredth dollars <br />{$33.75) ' per month per. employee. <br /> <br />Section 15. Retirement <br /> <br />The City shall amend its PEAS Local Miscellaneous <br />Member Retirement Plan for the purpose of adopting the <br />PIERS "credit for unused sic}: leave option." <br /> <br />Effective March 1, 1982, the city shall pay the <br />employee's contribution to the Public Employees' <br /> <br />-21- <br /> <br /> <br />
