13.5 Maternity Leave <br /> <br />The Appointing Authority may grant, upon written request, <br />maternity leave without pay for up to six (6) months <br />upon determination that such leave will not adversely <br />effect the needs of the City. <br /> <br />Section 14. Health and Welfare <br /> <br />14.1 <br /> <br />Hospital and Medical <br /> <br />Effective March 1, 1984, the City shall contribute an <br />amount not to exceed one hundred forty seven dollars and <br />eighty-two cents ($147.82) per month per employee to <br />provide health care coverage to employees and their <br />eligible dependents under either of the present Blue <br />Cross or Kaiser "S" health insurance plans or the addi- <br />tional HMO health insurance plan referred to in the <br />following paragraph. Effective June 1, 1984, the City's <br />contribution shall be increased to an amount not to <br />exceed one hundred eighty two dollars and eleven cents <br />($182.11) per month per employee. <br /> <br />During the term of this agreement, the City agrees to <br />implement a health maintenance organization (HMO) health <br />plan. Said HMO health plan shall be in addition to the <br />present Kaiser "S" and Blue Cross health plans. The <br />City further agrees during the term of this agreement to <br />compare the present Blue Cross health insurance plan to <br />other health insurance plans. The City shall not be <br />obligated to take any action as a result of this com- <br />parison. <br /> <br />For the purpose of providing aforesaid health care <br />coverage to eligible regular part-time employees who <br />average between twenty (20) and twenty-nine (29) hours <br />per week, the City shall contribute an amount not to <br />exceed fifty percent (50%) of the City's monthly contri- <br />bution established for full time employees as set forth <br />in the paragraph above. For regular part-time employees <br />who average between thirty (30) and thirty-nine (39) <br />hours per week, the City shall contribute an amount <br />not to exceed seventy-five percent (75%) of the City's <br />monthly contribution established for full time employees <br />as set forth in the paragraph above. <br /> <br />The City shall contribute for the purposes of providing <br />aforesaid hospital and medical coverage to employees <br />who retire after said date an amount not to exceed <br />Seventy-five Dollars ($75.00) per month for a period not <br />to exceed twelve (12) months from the date the employee <br />retires from the City. <br /> <br />-2Q- <br /> <br /> <br />
