EMERGENCY MEDICAL SERVICES AGREEMENT <br /> <br /> This Agreement is made and entered into this day of , <br /> <br /> 19 , by and between the COUNTY OF ALAMEDA, hereinafter referred to as <br /> "COUNTY" and the CITY OF PLEASANTON, hereinafter referred to as "CITY." <br /> <br /> WITNESSETH: <br /> <br /> WHEREAS, the COUNT~ has established an Emergency Medical Service <br /> District (EM-1983-1) to provide emergency paramedic ambulance service within <br /> its jurisdiction; <br /> <br /> WHEREAS, the territory of the City of Pleasanton is within the <br /> Emergency Medical Service District; and <br /> <br /> WHEREAS, the CITY is willing to provide first response to requests <br /> for emergency medical assistance in order to perform emergency first aid and <br /> basic life support prior to arrival of the ambulance; <br /> <br /> NOW, THEREFORE, IT IS HEREBY MUTUALLY AGREED AS FOLLOWS: <br /> <br />1. The CITY through its fire department shall respond according to its <br /> <br />custom and practices to requests for emergency medicalservices within its <br />jurisdiction, which are determined by CITY'S or other emergency dispatch <br />center to be potentially life threatening. <br /> <br />2. The CITY shall undertake to train and cause to be certified as <br /> <br />Emergency Medical Technicians - I (EMT-I) personnel who are regularly <br />dispatched as first responders on medical emergencies. <br /> <br />3. The CITY shall adhere to reasonable medical protocols established by <br /> <br />the County Itealth Officer. <br /> <br />4. The CITY agrees to complete a First Responder Prehospital Care Report <br /> <br />on a form provided by the COUNTY for each emergency medical response. One <br />copy of the report shall be given to the ambulance crew to accompany the <br />patient to the hospital, one copy shall be maintained by the CITY, and one <br />copy shall be provided to the COUNTY. <br /> <br />5. The CITY agrees to participate in meetings and committees sponsored <br /> <br />by COUNTY related to EMS activities. <br /> <br /> <br />
