Health and Welfare Coverage <br /> <br />15.1 Hospital and Medical Care - Xc~ive ~mployees <br /> <br /> Effective July 1, 1986, the City shall contribute an <br /> amount not to exceed two hundred eleven dollars and <br /> thirty-two cents ($211.32) per employee per month to <br /> provide health care coverage to employees and their <br /> eligible dependents under either of the present Blue <br /> Cross, Kaiser "S" or Foundation Health care plans. <br /> <br />15.2 Hospital and Medical Care - Retired Employees <br /> <br /> Effective July 1, 1986 for employees having retired for <br /> service on or after their fiftieth (50th) birthday the <br /> City shall reimburse upon receipt of proper <br /> documentation an amount not to exceed seventy five <br /> dollars ($75.00) per month toward the cost of self <br /> procured medical plan coverage. Said reimbursement <br /> shall cease on the employsets seventieth (70th) <br /> birthday. <br /> <br />15.3 Dental <br /> <br /> Effective July 1, 1986, the City shall contribute an <br /> amount not to exceed thirty-eight dollars and thirty <br /> five cents ($38.35) per month per employee to provide <br /> dental care benefits under the City~s present Bankerrs <br /> Life Nebraska Plan for the individual employee and <br /> eligible dependents. <br /> <br /> 15.4 Life Insurance <br /> Effective July 1, 1986, th~ City shall contribute an <br /> amount necessary to provide life insurance coverage <br /> under the present Standard Insurance group policy for <br /> the individual employee. <br /> <br /> - 20 - <br /> <br /> <br />
