15.5 :Long-~erm Dis~bilit~ <br /> <br /> ~ffective ~uly l, 1984, the ~ity shall contribute an <br /> amount necessary to provide long-term disability <br /> insurance under the City;s present Canada Life Group <br /> Policy for the individual employee~ provided, however, <br /> that in no event shall the City be obligated to <br /> contribute an amount in excess of one dollar and fifty <br /> cents ($1.50) per one hundred dollars ($100) of monthly <br /> salary of the first $2250 of monthly salary not to <br /> exceed a total contribution of thirty-three dollars and <br /> seventy-five cents ($33.75) per month per employee. <br /> <br />'15.6 Additional Cit~ Contributions to Health and Welfare <br /> Insurance Premiums <br /> <br /> To offset any increases in premiums occurring during the <br /> first year of this agreement, the City will increase its <br /> combined maximum contribution toward the 'monthly <br /> premiums of health, dental, life and long-term <br /> disability insurance by an amount not to exceed ten <br /> dollars ($10.00) per month. The specific division of <br /> said ten dollars ($10.00) between the various insurances <br /> shall be by mutual agreement of the Union and the City. <br /> <br /> 15.7 Substitution of Health and Long-Term Disabilit~ Plans <br /> <br /> During the second year of this agreement, the City and <br /> Union agree to meet to consider the substitution of <br /> another health insurance plan for one of the present <br /> health insurance plans offered by the City and the <br /> substitution of another long-term disability insurance <br /> plan for the present longrterm disability insurance <br /> plan. Any decision to substitute one or both plans <br /> shall be by mutual agreement made solely on the basis of <br /> the merits of the substitute plans. Merits of the plan <br /> shall include plan costs, benefits, rate history, <br /> reputation of the carrier, and administrative <br /> requirements. Any decision to substitute health <br /> insurance plans must be made before April 1, 1987. Any <br /> decision to substitute long-term disability insurance <br /> plans must be made prior to February 1, 1987. <br /> <br /> - 21 - <br /> <br /> <br />
