12.2 Dentml <br /> <br /> Effective January 1, 1986, the City shall contribute an <br /> amount necessary to provide dental care benefits under <br /> the present Bankers Life Nebraska Plan for the <br /> individual employee and eligible dependents; provided, <br /> however, that in no event shall the City be obligated to <br /> contribute an amount in excess of Thirty Eight Dollars <br /> and Thirty Five Cents ($38.35) per month per employee to <br /> provide the ~foresaid coverage. <br /> <br />12.3 nife~ Accidental Death & Dismemberment and Long Term <br /> Disability <br /> <br /> Effective January 1, 1986 the City shall contribute an <br /> amount necessary to provide life, accidental death and <br /> dismemberment, and long-term disability insurance under <br /> the City's present Canada Life Insurance Company Group <br /> <br /> policy or agreed upon alternative policy for the <br /> individual employee; provided, however, that in no event <br /> shall the City be obligated to contribute an amount in <br /> excess of One Dollar and Fifty Cents ($1.50) per One <br /> Hundred Dollars ($100.00) of monthly salary of the first <br /> $2,250.00 of monthly salary not to exceed a total <br /> contribution of Thirty Three Dollars and Seventy Five <br /> Cents ($33.75) per month per employee. <br /> <br />Section 13. Retirement <br /> <br />13.1 Payment Employee Contribution <br /> <br /> Effective January 1, 1982, the City shall pay the <br /> employee's contribution to the Public Employees' <br /> Retirement System (PERS). The amount of said City <br /> payment shall equal but not exceed nine percent (9%) of <br /> the employee's compensation subject to retirement <br /> contributions. <br /> <br />13.2 Adoption of PERS Single Highest Year Option <br /> <br /> Upon the affirmative majority vote of the voting <br /> membership of the Association, the city agrees during <br /> the 1986 or 1987 calendar year to modify its contract <br /> with PERS for the purpose of adopting the "single <br /> highest year" option. The total cost of said option <br /> shall be paid by the employee. However, should the <br /> majority of the Association voting members elect, the <br /> cost of said option may be included in the calculation <br /> of the 95th percentile as set forth in Section 5.1 of <br /> this memorandum of understanding. <br /> <br />Memorandum of Understanding <br />Page 13 <br /> <br /> <br />
