11.4 Military Leave <br /> <br /> Military leave shall be granted in accordance with the <br /> provisions of law. All employees entitled to military <br /> leave shall give the Appointing Authority an opportunity <br /> within the limits of military regulations to determine <br /> when such leave shall be taken. <br /> <br /> 11.5 Maternit~ Leave <br /> <br /> The Appointing Authority may grant, upon written <br /> request, maternity leave without pay for up to six (6) <br /> months upon determination that such leave will not <br /> adversely effect the needs of the City. <br /> <br />Section 12. Health and Welfare Coverage <br /> <br /> 12.1 Hospital and Medical <br /> <br /> Effective January 1, 1986, the City shall contribute an <br /> amount necessary to provide hospital and medical care <br /> benefits under either the present Blue Cross Plan, <br /> Kaiser S-1 Plan or, Foundation Health Plan I for the <br /> individual employee and a portion of the amount <br /> necessary to provide coverage for eligible dependents; <br /> provided, however, that in no event shall the City be <br /> obligated to contribute an amount in excess of Two <br /> Hundred One Dollars and Thirty Six Cents ($201.36). <br /> <br /> Effective May 1, 1986, the City shall contribute an <br /> amount necessary to provide hospital and medical care <br /> benefits under either the present Blue Cross Plan, <br /> Kaiser S-1 Plan or Foundation Health Plan I for the- <br /> individual employee and a portion of the amount <br /> necessary to provide coverage for eligible dependents; <br /> provided, however, that in no event shall the City be <br /> obligated to contribute an amount in excess of Two <br /> Hundred Eleven Dollars and Thirty Two Cents ($211.32) <br /> per month per employee to provide the aforesaid <br /> coverage. <br /> <br /> For the purpose of providing the aforementioned medical <br /> coverage to employees who retire for service on or after <br /> their fiftieth (50th) birthday, the City shall <br /> contribute an amount equal to one half (1/2) the monthly <br /> premium for said health coverage; in no event, however, <br /> shall said City contribution exceed Fifty Dollar <br /> ($50.00) per month per employee.
