Exposed (unknown type) wires at locker and large room adjacent to the mat room. Clear <br />area for access and schedule an inspection with the building division to determine required <br />action. (Permit may be required) 2001 California Electrical code, section 300-4. <br />7. Non-functioning GFCI outlets and reverse polarity (improper wiring within box and/or <br />system) at several outlets. Replace andlor repair. This work may be covered under the <br />tenanf improvement application. 2001 California electrical code, section 110-3(b). <br />8. Shower installed without proper permits and or inspections. As evident by the opening at the <br />showerhead and back wall. (Permit required). Dublin Municipal Code, section 7.28.280 <br />and 7.40.060. <br />9. An apparent un-authorized use of space -Plans and building type support an B, M or A-3 <br />occupancy type, although, conditions found indicate the space was being used for residence <br />and/or sleeping purpose. This was evident by the following: <br />a. An abundance of clothing throughout the space <br />b. Food products throughout the kitchen and large gathering area. (Kitchen, which <br />includes a washer and dryer hook up, installed without proper permits or inspections.) <br />c. Personnel affects throughout the space, i.e., personnel hygiene products, pistols, <br />shoes, books etc. <br />d. Couch (bed inside) in the large room next to mat room. <br />e. Statements from the Police, i.e., pervious conversation with you, where you stated to <br />the Officers that 7233 Regional Street was your residency and the DMV lists your <br />address as 7233 Regional Street. <br />Due to the condition of the commercial space, specifically, items 1, 2, 3, 4 and 9. The Citv of Dublin <br />Building Division has declared this tenant space un-safe and therefore has revoked occupancy. <br />The local utilities may be contacted for purposes of having the utilities removed. Occupancy will <br />not be release until the tenant space has been returned to the minimum requirements of the Building <br />Codes. <br />The remaining items listed above, specifically, 5, 6, 7 and 8 will need corrected within 30 after <br />occupancy release. <br />You aze required to obtain all applicable permits from the City of Dublin Building Division in order to <br />correct all noted deficiencies. You may have your contractor obtain the permits or you may apply for the <br />permits yourself pursuant to all applicable laws and regulations. <br />The above items are not a complete list of the requirements listed in the conditional use permit. You are <br />hereby ordered to obtain the building permit to correct all items under the conditional use permit within <br />60 days and to complete all work within 120 days. On December 10, 2003 the Building Division sent <br />you corrective comments from your plan check you had previously submitted to the City for review and <br />subsequent permit issuance. As of today the City has not received a response from you nor has the City <br />received your returned corrected plans for additional review. <br />Page 2 of 4 <br />
