employees who average between thirty (30) and thirty-nine (39) <br /> hours per week, the Ciky shall contribute an amount not to <br /> exceed seventy-five percent (75%) of the City's monthly <br /> contributiol~ established for full time employees as set forth <br /> in the paraIjraph above. <br /> <br /> Effective March 1, 1986 the city shall contribute an amount not <br /> to exceed seventy-five dollars and no cents ($75.00) per month <br /> for the purpose of providing aforesaid hospital and medical <br /> coverage to employees who, having reached the age-of sixty (60) <br /> with twenty (20) years of City employment, retire "for <br /> service." Said contribution shall continue until the employee <br /> reaches his or her seventy second (72nd) birthday. Effective <br /> January 1, 1988 the amount will be increased to an amount not <br /> to exceed l~inety dollars and no cents ($90.00) per month per <br /> employee. <br /> <br />14.2 Dental <br /> <br /> Effective March 1, 1987, the City shall contribute an amount <br /> not to exceed forty one dollars and eighty eight cents ($41.88) <br /> per month [~er employee to provide dental care benefits under <br /> the City's present Banker's Life Nebraska Plan for the <br /> individual employee and eligible dependents. <br /> <br /> The City's maximum monthly contribution shall be increased to <br /> include any increase in the plan's monthly premium occurring <br /> during the term of this agreement. <br /> <br />14.3 Life, Accidental Death & Dismemberment and Long-Term Disability <br /> <br /> The City shall contribute an amount necessary to provide <br /> $20,000 life and $20,000 accidental death and dismemberment <br /> insurance coverage for the individual full time employee. <br /> <br /> Effective March 1, 1987, the City shall contribute an amount <br /> necessary to provide long-term disability insurance under the <br /> present Canada Life group policy or an acceptable alternative <br /> plan for the individual full time employee; provided, however, <br /> that in no event shall the City be obligated to contribute an <br /> amount in excess of one dollar and fifty six and three tenths <br /> cents ($1.563) per one hundred dollars ($100) of monthly salary <br /> of the first $2,250 of monthly salary not to exceed a total <br /> contribution of thirty-five and seventeen one hundredth dollars <br /> ($35.17) per month per employee. <br /> <br /> Effective September 1, 1987, the maximum covered salary shall <br /> be increased to $2,500. Any increase in monthly premiums <br /> resulting from the increase in covered salary shall be paid by <br /> the City. <br /> <br />14.4 Health and Welfare Benefit Continuation <br /> <br /> Employees off work due to verifiable personal illness and <br /> having exhausted their accrued sick leave benefits set forth in <br /> Section 12 shall continue their eligibility for health and <br /> welfare payments in accordance with Section 14.
