approved leave, the employee shall be reinstated in the <br /> position held at the time leave was granted. Failure on the <br /> part of an employee on leave to report promptly at its <br /> expiration or within a reasonable time after notice to return <br /> to duty shall be cause for discipline. Department Heads may <br /> grant a permanent or probationary employee leave of absence <br /> without pay for not to exceed one (1) calendar week. Such <br /> leaves shall be reported to the Personnel Officer. <br /> <br />13.2 Jury Leave .... <br /> <br /> Every employee of the City who is called or required to serve <br /> as a juror shall be entitled to leave from his/her duties with <br /> the City during the period of such service or while necessarily <br /> being present in court as a result of such call. The employee <br /> shall be paid the difference between his/her full salary and <br /> any payment received by him/her, except travel pay, for such <br /> duty. <br /> <br />13.3 Leaves Resulting from Subpoena <br /> <br /> Leave of absence with pay shall be granted to a person while <br /> going to and from court and answering a subpoena as a witness. <br /> The employee shall be paid the difference between his/her full <br /> salary and any payment received by him/her, except travel pay, <br /> for such duty. <br /> <br />13.4 Military Leave <br /> <br /> Military leave shall be granted in accordance with the <br /> provisions of law. All employees entitled to military leave <br /> shall give the Appointing Authority an opportunity within the <br /> limits of military regulations to determine when such leave <br /> shall be taken. <br /> <br />13.5 Maternity Leave <br /> <br /> The Appointing Authority may grant, upon written request, <br /> maternity leave without pay for up to six (6) months upon <br /> determination that such leave will not adversely effect the <br /> needs of the City. <br /> <br />Section 14. Health and Welfare <br /> <br />14.1 Hospital and Medical <br /> <br /> The City shall contribute an amount not to exceed two hundred <br /> twenty one dollars and sixty five cents ($221.65) per month per <br /> employee to provide health care coverage to employees and their <br /> eligible dependents under either of the present Blue Cross, <br /> Kaiser "S" or Foundation health insurance plans. <br /> <br /> For the purpose of providing aforesaid health care coverage to <br /> eligible regular part-time employees who average between twenty <br /> (20) and twenty-nine (29) hours per week, the City shall <br /> contribute an amount not be exceed fifty percent (50%) of the <br /> City's monthly contribution established for full time employees <br /> as set forth in the paragraph above.
