8. A description of any plan to subcontract any portion of the work to an individual or <br />organization other than yourself. The description must include the type and length of work, <br />name of the subcontractor(s), and the names, titles, and qualifications of all individuals <br />performing project work for the subcontractor. <br />9. A detailed fee schedule including your preferred invoicing procedures. The City anticipates <br />a "not to exceed" fee for this project. An open ended hourly based fee proposal will be <br />determined to be incomplete. <br />c) Supalemental Material <br />You may provide any material not specifically required as supplemental information. Additional <br />material may include the following: <br />1. Additional reports and/or descriptions of similar projects you have completed. <br />2. Promotional material describing your products and services. <br />3. Additional references. <br />VI. PROPOSAL SUBMISSION <br />a) Authorization <br />If a proposal is being made by an individual, it must be signed with your full name including <br />your address. If a proposal is being made by a partnership, it must be signed with a partnership <br />name and by the authorized general partner. If approval is being made by a joint venture, it must <br />be signed with the full name and address of each member of the joint venture. If approval is <br />being made by a corporation, it shall be signed by the president and the secretary in the corporate <br />name and the corporate seal shall be affixed. <br />b) Proposal Delivery <br />Proposals must be submitted in a sealed envelope, plainly marked with your company name, <br />address, and telephone number, with "Proposal for LPFD SCBA Program" clearly visible on the <br />exterior. It is your sole responsibility to see that your proposal is received before the submission <br />deadline. You bear all risks and delays associated with the U.S. mail. <br />All proposals must be submitted to the City of Pleasanton, City Clerk's Office, 123 Main <br />Street, P.O. Box 520, Pleasanton, CA 94566 no later than 4:00 p.m. on October 5, 2007. <br />VII. SELECTION PROCESS AND EVALUATION CRITERIA <br />a) Evaluation Criteria <br />12 <br />
