b) Vendors shall provide the Livermore Pleasanton Fire Department with NFPA 1981, 2007 <br />Edition compliant, SCBAs as specified in the scope of request at the earliest possible opportunity <br />but not later than May 1, 2008. <br />c) Vendors shall adequately train a minimum of three Livermore-Pleasanton Fire Department <br />SCBA Service Technicians to adequately test and maintain the specified SCBAs within 60 days <br />of delivery of the NFPA 1981, 2007 Edition SCBAs as specified in the scope of request. <br />V. REQUIRED INFORMATION <br />a) Proposal preparation <br />All responses to this RFP but must be made in accordance with the specifications as set forth <br />herein. Failure to adhere to any specifications contained herein, maybe cause to reject your <br />proposal. <br />b) Ability to Perform <br />As part of your proposal, you must present satisfactory evidence indicating your ability to meet <br />the scope of work detailed in this RFP. Further, you must provide adequate pricing and service <br />information to assure that the fees for services and products are clearly described. To this end, <br />your proposal must include the following information: <br />1. Your name, address, telephone number and the name of your company. <br />2. If appropriate, the names, business address and telephone numbers of your company's <br />officers, directors, and associates and the names and addresses of any parent of subsidiary of <br />your company. The proposal must describe the nature of the work and the line of authority <br />of these individuals and/or companies as they relate to this project. <br />3. A listing of your experience related to work included in this IZFP. Experience with <br />municipalities is desirable but not required. <br />4. A minimum of three related business references, including their names, addresses, and phone <br />numbers, plus a description of the type of work you performed for them. <br />5. A detailed work plan describing your approach to managing and coordinating this project. <br />The description should, at a minimum, include all tasks listed in Section II, "Scope of Work." <br />In addition, you should include any steps/tasks not included in the "Scope of Work" that you <br />think would materially affect the quality of the project. The quality and thoroughness of the <br />proposer's work plan is an evaluation criterion. <br />6. The resume(s) of all individuals performing work on the project including identification of <br />the project manager. <br />7. Your time commitment to the project. <br />11 <br />
