Section 12. Health and Welfare Coverage <br /> <br />12.1 Hospital and Medical <br /> <br /> Effective January 1, 1988, the City shall contribute an <br /> amount necessary to provide hospital and medical care <br /> benefits under either the present Blue Cross Plan, <br /> Kaiser S-1 Plan or, Foundation Health Plan I for the <br /> individual employee and a portion of the amount <br /> necessary to provide coverage for eligible dependents; <br /> provided, however, in no event shall the City be <br /> obligated to contribute a monthly amount in excess of <br /> the monthly premium for the Kaiser S-1 Health Plan full <br /> family coverage. <br /> <br /> For the purpose of providing health coverage to <br /> employees who retire for service (not disability) with a <br /> minimum of ten years of city service, the city shall <br /> contribute the sum of Seventy Five dollars ($75.00) <br /> monthly. Said contribution shall cease upon the <br /> employee's sixty fifth (65th) birthday. <br /> <br />12.2 Dental <br /> <br /> Effective January 1, 1988, the City shall contribute an <br /> amount necessary to provide dental care benefits under <br /> the present Bankers Life Nebraska Plan for the <br /> individual employee and eligible'dependents; provided, <br /> however, that in no event shall the City be obligated to <br /> contribute an amount in excess of Forty Two Dollars <br /> ($42.00) per month per employee to provide the aforesaid <br /> coverage. <br /> <br /> Effective January 1, 1989, said maximum contribution <br /> shall be increased to Forty Five Dollars ($45.00) per <br /> month. <br /> <br />12.3 Life, Accidental Death & Dismemberment and Long Term <br /> Disability <br /> <br /> Effective January 1, 1988, the City shall contribute an <br /> amount necessary to provide life, accidental death and <br /> dismemberment, and long-term disability insurance under <br /> the City's present Canada Life Assurance Company Group <br /> policy or agreed upon alternative policy for the <br /> individual employee; provided, however, that in no event <br /> shall the City be obligated to contribute an amount in <br /> excess of One Dollar and Seventy Three and Six-tenths <br /> cents ($1.736) per One Hundred Dollars ($100.00) of <br /> monthly salary of the first $2,250.00 of monthly salary <br /> not to exceed a total contribution of Thirty Nine <br /> Dollars and Five Cents ($39.05) per month per employee. <br /> <br />Memorandum of Understanding <br />Page 12 <br /> <br /> <br />
