Upon qualifying for and receiving parts A and B of Medicare, the City shall not be obligated to contribute a monthly amount in excess of the monthly premium for the Kaiser S-1 supplement to medicare health plan for employee and one dependent. In the case where the eligible employee is deceased, the amount the the City is obligated to pay shall be reduced by 1/2 (one half). If the spouse remarries, the obligation by the City shall terminate. The City reserves the right to meet and confer with the Association for the purposes of coordinating retired employee's hospital and medical care in the event Federal or State legislation is implemented that effects retiree's medical insurance. The City will contact the health care providers and seek the opportunity to place retirees under the active plans. 14.3 Health and Welfare Trust During the term of the Memorandum of Understanding, the City, upon request by the PCEA, will meet and confer with the Association regarding establishing a Health and Welfare Trust. Should the parties agree to the establishment of such a Trust, the City's contributions on behalf of the employees' will not exceed the amount contributed by the City toward the employee's current medical, dental and life insurance coverage at the time the Trust was established. 14.4 Dental Effective March 1, 1988, the City shall contribute an amount not to exceed forty one dollars and eighty eight cents (41.88) per month per employee to provide dental care benefits under the City's present and Banker's Life Nebraska Plan for the individual employee and eligible dependents. Effective March 1, 1989 said contribution shall increase to an amount not to exceed $45.00 per month per employee. In the event the insurance carrier is able to provide regular part-time employees dental insurance coverage, the City will make such coverage available on the same basis as the medical insurance is provided for part-time employees. 14.5 Life, Accidental Death & Dismemberment and Long-Term Disability The City shall contribute an amount necessary to provide $20,000 life and $20,000 accidental death and dismemberment insurance coverage for the indivdual full time employee. Effective March 1, 1988, employees may purchase, at the employees own expense and within the limits set by the insurance carrier, additional amounts of life insurance under the existing policy. - 26 -
