Upon qualifying for and receiving parts A and B of Medicare, the <br /> City shall not be obligated to contribute a monthly amount in <br /> excess of the monthly premium for the Kaiser S-1 supplement to <br /> medicare health plan for employee and one dependent. In the <br /> case where the eligible employee is deceased, the amount the <br /> the City is obligated to pay shall be reduced by 1/2 (one half). <br /> If the spouse remarries, the obligation by the City shall <br /> terminate. <br /> <br /> The City reserves the right to meet and confer with the <br /> Association for the purposes of coordinating retired employee's <br /> hospital and medical care in the event Federal or State <br /> legislation is implemented that effects retiree's medical <br /> insurance. <br /> <br /> The City will contact the health care providers and seek the <br /> opportunity to place retirees under the active plans. <br /> <br />14.3 Health and Welfare Trust <br /> <br /> During the term of the Memorandum of Understanding, the City, <br /> upon request by the PCEA, will meet and confer with the <br /> Association regarding establishing a Health and Welfare Trust. <br /> Should the parties agree to the establishment of such a Trust, <br /> the City's contributions on behalf of the employees' will not <br /> exceed the amount contributed by the City toward the employee's <br /> current medical, dental and life insurance coverage at the time <br /> the Trust was established. <br /> <br />14.4 Dental <br /> <br /> Effective March 1, 1988, the City shall contribute an amount not <br /> to exceed forty one dollars and eighty eight cents (41.88) per <br /> month per employee to provide dental care benefits under the <br /> City's present and Banker's Life Nebraska Plan for the individual <br /> employee and eligible dependents. <br /> <br /> Effective March 1, 1989 said contribution shall increase to an <br /> amount not to exceed $45.00 per month per employee. <br /> <br /> In the event the insurance carrier is able to provide regular <br /> part-time employees dental insurance coverage, the City will make <br /> such coverage available on the same basis as the medical <br /> insurance is provided for part-time employees. <br /> <br />14.5 Life, Accidental Death & Dismemberment and Long-Term Disability <br /> <br /> The City shall contribute an amount necessary to provide $20,000 <br /> life and $20,000 accidental death and dismemberment insurance <br /> coverage for the indivdual full time employee. <br /> <br /> Effective March 1, 1988, employees may purchase, at the employees <br /> own expense and within the limits set by the insurance carrier, <br /> additional amounts of life insurance under the existing policy. <br /> <br /> - 26 - <br /> <br /> <br />
