Box 520 <br /> Pleasanton, California 94566-0802 <br /> (415) 484-8114 <br /> <br />Greetings: <br /> <br />In 1989 the Pleasanton Fire Department began an innovative program to insure <br />annual fire and life safety inspections in all businesses. This program allows <br />certain businesses to conduct their own inspections. <br /> <br />The inspection is to be conducted by the person in the highest authority at the <br />business, and the forme returned to the Pleasanton Fire Department by the date <br />noted on the front of the form. We recommend conducting the inspection as soon <br />as possible so that, should any corrections be required, you can complete them <br />and still return your forms in the time allowed. <br /> <br />Please return only the original (white copy) of the inspection form; the yellow copy <br />is for your records. <br /> <br />Be sure to include your business name and address at the top of the inspection <br />form. Correct all deficiencies before you return the form, and note that <br />information in the appropriate section. Random spot checks will be conducted to <br />ensure compliance <br /> <br />Insert the date your fire extinguisher was last serviced and the name of the <br />company performing the service (see question #19). Instructions for locating this <br />information are listed on the enclosure included with this letter. Fire <br />extinguishers are required to be serviced annually. If yours is due now or within <br />the next three months, please have this done prior to returning the forms and <br />note the information in question # 19. <br /> <br />Be sure to read the instruction sheet before completing the form. If you need <br />assistance or have any questions, please call 484-8114 prior to the thirty (30) day <br />deadline. Failure to respond will result in an inspection conducted by a member <br />of the fire department.
