Owner, manager or person in highest authority on the premises should <br /> conduct the inspection and sign the form. <br /> <br /> 2. Print (or correct) your name and address in the blanks provided at the top of <br /> page 1. <br /> <br /> 3. Read all the instructions on the top of the worksheet. <br /> <br /> 4. With work sheet in hand, walk through your business and answer each <br /> question. Fill in all applicable blanks. <br /> <br /> 5. After you have answered all questions AND MADE ALL NECESSARY <br /> CORRECTIONS, read the statement in bold type at the end of the worksheet <br /> on page three (3). Sign and print your name and enter the date on the <br /> appropriate lines. <br /> <br /> 6. MAKE ALL NECESSARY CORRECTIONS PRIOR TO RETURNING THE <br /> FORM. Forms must be returned no later than thirty (30) days from the issue <br />- date (see top of page one). <br /> <br /> 7. If you require any assistance or have any questions, call (415) 484-8114 prior <br /> to the 30-day deadline. <br /> <br /> BUSINESS AND EMERGENCY RECORD <br /> <br /> 1. This information is valuable to the fire department in the event of an after- <br /> business hours emergency should entrance to your business be required or <br /> we need to contact you for such an emergency. <br /> <br /> FIRE EXTINGUISHERS <br /> <br /> 1. A minimum of one 2A-IOBC portable fire extinguisher is required in every <br /> occupancy. Check the label on your extinguisher to determine correct size (a <br /> 1A-IOBC extinguisher is not acceptable to fill the requirement). <br /> <br /> 2. Fire extinguishers are required to be inspected or serviced annually by a <br /> company licensed by the State Fire Marshal. If yours has not been serviced <br /> within the last year, call a licensed company (in the yellow pages under <br /> "FIRE'~) and have it serviced PRIOR to completing question # 17. The date <br /> and name of the company performing the service must be filled in an <br /> question #17. <br /> <br /> P~ge 2 <br /> <br /> <br />
