When the forms are delivered, the business responsibles are told that spot checks
will be conducted to determine compliance and that they may request assistance
with the inspection if they so desire. Deliberate falsification of the forms is
considered perjury, and this information will be noted on the worksheet which
must be signed by the person conducting the inspection. Falsifying the forms can
result in the issuance of a citation.

A record of which occupancies have received forms will be kept on a monthly
basis on the department's computer system. Each year in the same month, new
forms will be sent to the businesses for a repeat inspection. Eventually all of these
small businesses will conduct self-inspections annually.

Not included in the self-inspection program are public assemblies, occupancies
requiring permits under the fire code or Hazardous Materials Ordinance,
schools, churches, sprinklered buildings, occupancies with fixed fire protection
systems and state fire marshal-licensed facilities. Inspections on these
occupancies will be conducted by the bureau.

The self inspection program frees engine companies to concentrate on inspections
where they are more needed - high hazard, high life-loss industrial areas and
reduces the turn-over to yearly. The self-inspection program assumes that the
business community has an interest in fire protection and can be trained and
encouraged to take an active part.

Page 2
