Upon qualifying for and receiving Parts A and B of Medicare, <br />the city shall not be obligated to contribute a monthly amount <br />in excess of the monthly premium for the Kaiser S-1 supplement <br />to Medicare Health Plan for employee and one dependent. In <br />the case where the eligible employee is deceased, the amount <br />the city is obligated to pay shall be reduced by 1/2 (one- <br />half). If the spouse remarries, the obligation by the city <br />shall terminate. <br />5.3 Dental Insurance <br />Management and confidential employees and their eligible <br />dependents shall be offered enrollment in the present Ameritis <br />Bankers Life Nebraska dental plan to include a maximum <br />lifetime orthodontia benefit of $1,000. <br />5.4 Life Insurance. <br />For the purpose of providing life and accidental death and <br />dismemberment insurance to management and confidential <br />employees, the city shall provide coverage equal to twice the <br />employee's annual salary to a maximum coverage of one hundred <br />thousand dollars ($100,000). The total monthly premium for <br />said coverage shall be paid by the city. Management and <br />confidential employees may, at their own expense and within <br />limits set by the insurance carrier, purchase additional <br />amounts of life insurance coverage. <br />5.5 Long-Term Disability Insurance. <br />The city shall provide management and confidential employees <br />long term disability insurance coverage at no cost to the <br />employee. Effective October 1, 1989 the maximum monthly <br />disability insurance benefit shall be increased from $1,500.00 <br />to $3,000.00. <br />5.6 Medical Reimbursement Program. <br />The city shall implement a program whereby employees electing <br />to forego dependent health coverage may be reimbursed within <br />the limits established in the program for certain medical <br />expenses allowable under the Internal Revenue Code. <br />- 7 - <br />
