Upon qualifying for and receiving parts A and B of Medicare, the City shall not be obligated to contribute a monthly amount in excess of the monthly premium for the Kaiser S-1 supplement to medicare health plan for employee and one dependent. In the case where the eligible employee is deceased, the amount the City is obligated to pay shall be reducedby 1/2 (one half). If the spouse remarries, the obligation by the City shall terminate. The City reserves the right to meet and confer with the Association for the purposes of coordinating retired employso's hospital and medical care in the event Federal or State legislation is implemented that effects retireo's medical insurance. The City will contact the health care providers and seek the opportunity to place retirees under the active plans. 14.3 Dental The City shall amend its present Ameritas dental insurance plan effective May 1, 1992 for the purpose of increasing the maximum orthodontia coverage to $1,500.00. Effective May 1, 1992 the City shall contribute an amount not to exceed fifty two dollars and seventy four cents ($52.74) per month to provide dental care benefits including the above orthodontia coverage under the City's Ameritas dental insurance plan for the individual employees and eligible dependents. Effective November 1, 1993 said maximum contribution shall be increased to fifty-six dollars and sixty-six cents ($56.66) per month. Effective JandafT 1, 1994 said ~axiJaum contribution shall be increased to sixty-one dollars and fifty-seven cents ($61.57) per month. Effective January 1, 1996 said maximum contribution shall be increased to sixty-eight dollars ($6S.00) per month. For the purpose of providing aforesaid dental care coverage for eligible regular part-time employees including job share employees who average between twenty (20) and twenty-nine (29) hours per week, the City shall contribute an amount not to exceed fifty percent (50%) of the city's monthly contribution established for full time employees as set forth above. For regular part-time employees including job share employees who average between thirty (30) and thirty-nine (39) hours per week, the City shall contribute an amount not to exceed seventy-five percent (75%) of the City's monthly contribution established for full-time employees as set forth above. 14.4 Life Insurance Effective May 1, 1992 the City shall contribute an amount necessary to provide $35,000 life and $35,000 accidental death and dismemberment insurance coverage for the individual full-time employee. - 19 -
