Upon qualifying for and receiving parts A and B of Medicare, the <br /> City shall not be obligated to contribute a monthly amount in <br /> excess of the monthly premium for the Kaiser S-1 supplement to <br /> medicare health plan for employee and one dependent. In the case <br /> where the eligible employee is deceased, the amount the City is <br /> obligated to pay shall be reducedby 1/2 (one half). If the spouse <br /> remarries, the obligation by the City shall terminate. <br /> <br /> The City reserves the right to meet and confer with the Association <br /> for the purposes of coordinating retired employso's hospital and <br /> medical care in the event Federal or State legislation is <br /> implemented that effects retireo's medical insurance. <br /> <br /> The City will contact the health care providers and seek the <br /> opportunity to place retirees under the active plans. <br /> <br />14.3 Dental <br /> <br /> The City shall amend its present Ameritas dental insurance plan <br /> effective May 1, 1992 for the purpose of increasing the maximum <br /> orthodontia coverage to $1,500.00. <br /> <br /> Effective May 1, 1992 the City shall contribute an amount not to <br /> exceed fifty two dollars and seventy four cents ($52.74) per month <br /> to provide dental care benefits including the above orthodontia <br /> coverage under the City's Ameritas dental insurance plan for the <br /> individual employees and eligible dependents. Effective November <br /> 1, 1993 said maximum contribution shall be increased to fifty-six <br /> dollars and sixty-six cents ($56.66) per month. Effective JandafT <br /> 1, 1994 said ~axiJaum contribution shall be increased to sixty-one <br /> dollars and fifty-seven cents ($61.57) per month. Effective <br /> January 1, 1996 said maximum contribution shall be increased to <br /> sixty-eight dollars ($6S.00) per month. <br /> <br /> For the purpose of providing aforesaid dental care coverage for <br /> eligible regular part-time employees including job share employees <br /> who average between twenty (20) and twenty-nine (29) hours per <br /> week, the City shall contribute an amount not to exceed fifty <br /> percent (50%) of the city's monthly contribution established for <br /> full time employees as set forth above. For regular part-time <br /> employees including job share employees who average between thirty <br /> (30) and thirty-nine (39) hours per week, the City shall contribute <br /> an amount not to exceed seventy-five percent (75%) of the City's <br /> monthly contribution established for full-time employees as set <br /> forth above. <br /> <br />14.4 Life Insurance <br /> <br /> Effective May 1, 1992 the City shall contribute an amount necessary <br /> to provide $35,000 life and $35,000 accidental death and <br /> dismemberment insurance coverage for the individual full-time <br /> employee. <br /> <br /> - 19 - <br /> <br /> <br />
