Employees may purchase, at the employees' own expense and within <br /> the limits set by the insurance carrier, additional amounts of life <br /> insurance under the existing policy. <br /> <br />14.5 Lona Term Disability Insuranc~ <br /> <br /> Effective May 1, 1992, the City shall contribute an amount <br /> necessary to provide long-term disability insurance under the <br /> present Canada Life group policy or an acceptable alternative that <br /> in no event shall the City be obligated to contribute an amount in <br /> excess of two dollars and sixteen cents ($2.16) per one-hundred <br /> dollars ($100) of monthly salary of the first $2,500 of monthly <br /> salary not to exceed a total contribution of fifty-four dollars <br /> ($54.00) per month per employee. <br /> <br />14.6 Health and Welfare Benefit Continuatio- <br /> <br /> ~mployees off work due to verifiable personal illness and having <br /> exhausted their accrued sick leave benefits set forth in Section 13 <br /> shall continue their eligibility for health and welfare payments in <br /> accordance with Section 14. Employees off work on leaves of <br /> absence without pay shall not be eligible for said health and <br /> welfare benefit payments. <br /> <br />Section 15 Retirement <br /> <br /> During the term of this contract, the city shall continue to <br /> provide the PERS "Credit for unused sick leave option" and to local <br /> miscellaneous members Section 21382.2 1959 survivor benefits. <br /> Effective February 1, 1993 the City shall amend its PERS contract <br /> for the purposes providing the "One Year Final Compensation Option" <br /> (Section 21382.2). <br /> <br /> Effective April 1, 1996 the City shall amend its contract with PERS <br /> for the purpose of providing the 2% @ 55 retirement option. <br /> <br /> Effective March 1, 1982, the City shall pay the employee's <br /> contribution to the PublicEmployee's Retirement System (PERS). The <br /> amount of said City payment shall equal, but not exceed seven <br /> percent (7%) of the employee's compensation subject to retirement <br /> contributions. <br /> <br /> For the one police clerk currently enrolled in the Public <br /> Employees' Retirement System's "public safety retirement program," <br /> the City shall pay the contribution to PERS. The amount of said <br /> City payment shall equal, but not exceed nine percent (9%) of the <br /> employee's compensation subject to retirement contribution. In <br /> recognition of this higher contribution by the City, the base <br /> monthly salaries paid this employee, effective May 2, 1992 shall be <br /> adjusted to provide the following: <br /> <br /> Police Clerk (1) $3208 <br /> <br /> <br />
