Exhibit A Page 5 5. Annually file with the Department of Parks and Community Services a roster containing the name, address and phone number of all officers and/or boardmembers. 6. Comply with the insurance requirements referred to in I.F. 7. Provide sufficient documentation to the City to ensure nonprofit status. 8. Provide the Director of Parks and Community Services or designee with general program information, in addition to special events. 9. Require all program participants to file a properly signed document holding the City of Pleasanton harmless in case of an accident. 10. Submit annually and upon request from the Director of Parks and Community Services an approved budget and a year-end financial statement. These annual financial reports are to coincide with the budget year of the sanctioned organization. The financial records of the sanctioned organization shall be open to the City and to any participants in the sanctioned activity. 11. Group membership must be comprised of at least 75% Pleasanton residents. 12. When utilizing City facilities, litter and trash must be removed from the site after each use. The City reserves the right to charge groups the cost of clean up, if necessary, due to noncompliance. B. Examples and Description of Sanctioned Groups These groups provide an opportunity for instruction and/or participation in a specific recreation activity open to the public. However, these activities are more limited in appeal or capacity than cosponsorship programs. (Examples: Soaring Club, Coin Clubs, Hobby Groups, Championship Youth Sports Programs.) C. Department Responsibilities and Services The Department may provide for sanctioned groups the following services: 1. The City will make recreational and park facilities available for the organizations without cost or at the lowest cost feasible.
