EXHIBIT A <br /> Sports Organization Co-Sponsorship Requirements and Responsibilities <br /> Requirements of a Co-Sponsorship Organization: <br /> • Offer recreational athletic program(s) that contribute positively to the community. The <br /> organization's program(s) must be accessible to the general public. The organization <br /> must ensure that the implementation of their programs align with the vision, mission, <br /> values and goals of the City of Pleasanton as stated in the One Pleasanton Strategic <br /> Plan and the Library and Recreation Department Strategic Plan. <br /> • Comprised of at least 75 participants. <br /> o If total participant enrollment falls below 75 participants, group will have one (1) <br /> calendar year to reach compliance. At the next submission of the roster <br /> enrollment, required 75 participant total must be met. <br /> ■ Should the organization fall under the required 75 total participants, Co- <br /> Sponsorship status will be revoked. <br /> ■ Organization(s) may apply for Co-Sponsorship status once the 75 total <br /> participants requirement is met. <br /> • Participant enrollment is to be comprised of at least 70% Pleasanton residents. <br /> o If participant enrollment falls below 70% Pleasanton residents, group will have <br /> one (1) calendar year to reach compliance. At the next submission of the roster <br /> enrollment, required residency must be met. <br /> • Should the organization fall under the required residency percentage, Co- <br /> Sponsorship status will be revoked. <br /> • Organization(s) may apply for Co-Sponsorship status once residency <br /> requirement is met. <br /> • Submit the organization's participant enrollment figures containing first and last name, <br /> address, and telephone number. <br /> o All addresses must be the physical location of the primary residence. P.O. box <br /> addresses will not be accepted. <br /> o Any participant listed on the roster with incomplete information will be considered <br /> a non-resident. <br /> o Participants will only be counted once towards residency. <br /> o Participants include only players registered and actively participating within the <br /> organization's program(s). Non-players such as coaches, officials, staff, <br /> volunteers, etc., shall not be included in the team counts or total number of <br /> organization participants for the purpose of allocating fields. <br /> • Submit the organization's board member roster containing name, address, and <br /> telephone number. <br /> o All addresses must be the physical location of the primary residence. P.O. box <br /> addresses will not be accepted. <br /> • Submit the Letter of Determination or Letter of Affirmation from the IRS that indicates its <br /> nonprofit 501 (c)(3) status. <br /> • Submit a copy of organization's endorsed or certified Articles of Incorporation <br /> demonstrating it is an active California Nonprofit Public Benefit Corporate Status. <br /> • Submit a copy of the organization's current financial statements on file (IRS Form 990 or <br /> 990EZ). <br /> • Comply with the City's Insurance Requirements. <br /> o Insured's name must match the name listed on the Field Use Application. <br /> o Minimum of$2,000,000 general liability insurance. The Commercial General <br /> Liability policy must have coverage on a per occurrence basis, including property <br />
