EXHIBIT A Sports Organization Co-Sponsorship Requirements and Responsibilities Requirements of a Co-Sponsorship Organization: • Offer recreational athletic program(s) that contribute positively to the community. The organization's program(s) must be accessible to the general public. The organization must ensure that the implementation of their programs align with the vision, mission, values and goals of the City of Pleasanton as stated in the One Pleasanton Strategic Plan and the Library and Recreation Department Strategic Plan. • Comprised of at least 75 participants. o If total participant enrollment falls below 75 participants, group will have one (1) calendar year to reach compliance. At the next submission of the roster enrollment, required 75 participant total must be met. ■ Should the organization fall under the required 75 total participants, Co- Sponsorship status will be revoked. ■ Organization(s) may apply for Co-Sponsorship status once the 75 total participants requirement is met. • Participant enrollment is to be comprised of at least 70% Pleasanton residents. o If participant enrollment falls below 70% Pleasanton residents, group will have one (1) calendar year to reach compliance. At the next submission of the roster enrollment, required residency must be met. • Should the organization fall under the required residency percentage, Co- Sponsorship status will be revoked. • Organization(s) may apply for Co-Sponsorship status once residency requirement is met. • Submit the organization's participant enrollment figures containing first and last name, address, and telephone number. o All addresses must be the physical location of the primary residence. P.O. box addresses will not be accepted. o Any participant listed on the roster with incomplete information will be considered a non-resident. o Participants will only be counted once towards residency. o Participants include only players registered and actively participating within the organization's program(s). Non-players such as coaches, officials, staff, volunteers, etc., shall not be included in the team counts or total number of organization participants for the purpose of allocating fields. • Submit the organization's board member roster containing name, address, and telephone number. o All addresses must be the physical location of the primary residence. P.O. box addresses will not be accepted. • Submit the Letter of Determination or Letter of Affirmation from the IRS that indicates its nonprofit 501 (c)(3) status. • Submit a copy of organization's endorsed or certified Articles of Incorporation demonstrating it is an active California Nonprofit Public Benefit Corporate Status. • Submit a copy of the organization's current financial statements on file (IRS Form 990 or 990EZ). • Comply with the City's Insurance Requirements. o Insured's name must match the name listed on the Field Use Application. o Minimum of$2,000,000 general liability insurance.
