Benefits for Medicare Eligible Retirees <br /> Retiree health benefits, including reimbursement, will cease once the employee <br /> becomes Medicare eligible. <br /> 12.3 Dental <br /> The City shall contribute an amount necessary to provide dental care benefits for the individual <br /> employees and eligible dependents. The City shall provide a lifetime maximum orthodontia coverage <br /> of$2,000 for individual employees and eligible dependents. <br /> 12.4 Life Insurance <br /> The City shall contribute an amount necessary to provide$100,000 life and $100,000 accidental death <br /> and dismemberment insurance coverage for individual full-time employees. <br /> Employees may purchase at their own expense and within the limits set by the insurance carrier, <br /> additional amounts of life insurance under the existing policy. <br /> 12.5 Vision Care <br /> A vision care plan will be provided as an option for employees and the cost of such coverage shall be <br /> included in the 125 Plan. <br /> 12.6. Long Term Disability <br /> The City shall pay to the Union the sum of twenty-nine dollars and fifty cents ($29.50), up to a <br /> maximum of forty dollars($40.00)towards the monthly premium for each Battalion Chief enrolled in <br /> the Union's long-term disability plan. Said payments shall be made in arrears monthly. <br /> 12.7 Extended Benefits <br /> The City will consider voluntary proposals from the Union's membership to utilize their leave and <br /> medical benefits to extend the leave and medical benefits for employees on protracted medical leave. <br /> 12.8 Employee Assistance Program <br /> The City shall provide an Employee Assistance Program for employees covered by this MOU. <br /> Section 13. Retirement <br /> 13.1 Retirement Plans and Employee Contributions <br /> A. Employees hired before December 22,2012 (Tier 1 Classic) <br /> Employees hired prior to December 22,2012 are provided the CalPERS 3%@ 50 local <br /> safety plan with the 12-month final compensation period. Such employees shall <br /> contribute 9.0%of their CalPERS reportable compensation towards the employee share <br /> and an additional 3.0% of their CalPERS reportable compensation towards the <br /> 4894-5387-2224 Q 24 <br />
