Documentation setting forth the date of issuance of the waiver, the 3183 <br />timeframe for the waiver; and, 3184 <br />iv. A record of the Tons of Organic Materials, Recyclable Materials, and Solid 3185 <br />Waste redirected to an alternative facility or Disposed at an Approved 3186 <br />Facility as a result of the waiver, recorded by Collection vehicle or Transfer 3187 <br />vehicle number/load, date, and weight. 3188 <br />b. A copy of the written notification provided to the City that documents the 3189 <br />approved material list that can be accepted at the Approved Organic Materials 3190 <br />Processing Site, in accordance with Section 5.13.3 of the Agreement. 3191 <br />c. Should the California Department of Food and Agriculture or the County 3192 <br />Agricultural Commissioner determine that some Organic Waste must be Disposed 3193 <br />of at a Solid Waste landfill as it cannot be safely recovered through the Approved 3194 <br />Processing Facilities, the Contractor shall provide a copy of the California 3195 <br />Department of Food and Agriculture approved compliance agreement for each 3196 <br />shipment stating that the material must be transported to a Solid Waste landfill 3197 <br />operating under the terms of its own compliance agreement for the pest or disease 3198 <br />of concern. This shall include documentation that describes the name of Generator, 3199 <br />date issued, location of final disposition, and the amount of Organic Waste that 3200 <br />was required to be disposed at a Solid Waste landfill. 3201 <br />d. A record of the total Tons of Discarded Materials, listed separately by Discarded 3202 <br />Material type, removed from illegal Disposal sites as part of an abatement activity, 3203 <br />listing each Collection event separately by date, location, and Tonnage Collected. 3204 <br />D. Marketing Plan. Contractor shall include an updated Recyclable Materials and Organic Materials 3205 <br />marketing plan in its annual report for City approval as required by Sections 5.13.2.D and 5.13.3.E. 3206 <br />E. Historical Data. (to be completed if Rates are to be determined using a cost-based adjustment 3207 <br />pursuant to Section 8.2) 3208 <br />Provide the following annual historical data for each Service Type: 3209 <br />1. Customer levels and subscription levels; 3210 <br />2. Solid Waste Tonnage; 3211 <br />3. Diversion Tonnage; 3212 <br />4. Gross Rate Revenues; and, 3213 <br />5. Material revenues for Recyclable Materials and Organic Materials by program type. 3214 <br />F. Variance Analysis. (to be completed if Rates are to be determined using a cost-based adjustment 3215 <br />pursuant to Section 8.2) 3216 <br />Provide the following variance analysis for each Service Type. For any variances greater than five 3217 <br />percent (5%) annually, Contractor shall provide sufficient rationale to support variance. 3218 <br />1. Variance analysis comparing current Rate Period to each of the prior Rate Periods of Agreement; 3219 <br />and, 3220 <br />2. Variance analysis comparing current Rate Period to each of the future projected Rate Periods. 3221
