Amounts shall be reconciled to the Related 3140 <br />Party Entity disclosures made in Contractor's annual audited financial statements referred to in 3141 <br />this Section; and, 3142 <br />2. At the City's request, Contractor shall provide the City with copies of working papers or other 3143 <br />documentation deemed relevant by the Contractor relating to information shown in the annual 3144 <br />disclosure letter. The annual disclosure letter shall be provided to the City within one hundred 3145 <br />twenty (120) calendar days after the end of the Contractor’s fiscal year. 3146 <br />C. Operational Information. In addition to requirements stated elsewhere in this Agreement, the 3147 <br />annual report shall include the following information: 3148 <br />1. Routes by Service Type 3149 <br />a. Number of routes per day; 3150 <br />b. Types of vehicles; 3151 <br />c. Crew size per route; 3152 <br />d. Number of full time equivalent (FTE) routes; 3153 <br />e. Number of accounts per route; 3154 <br />f. Total route hours per Service Type per year; and, 3155 <br />g. Average cost per route. 3156 <br />2. Personnel 3157 <br />a. Organizational chart; 3158 <br />b. Job classifications and number of employees (e.g. administrative, Customer 3159 <br />service representatives, drivers, supervisors, educational staff); 3160 <br />c. Wages by job classification; 3161 <br />d. Number of full time equivalents (FTE) positions for each job classification; and, 3162 <br />e. Number of hours per job classification per year. 3163 <br />3. Productivity Statistics 3164 <br />a. Number of accounts per Service Type; 3165 <br />b. Average number of setouts per Service Type; and, 3166 <br />c. Tons per route per day. 3167 <br />4. Maintenance 3168 <br /> a. Average cost per Service Type. 3169 <br />5. Operational Changes 3170 <br />a. Number of routes; 3171 <br />b. Staffing; 3172 <br />c. Supervision; and, 3173 <br />d. Collection services. 3174 <br />6. Disposal and Processing 3175 <br />a. If the Contractor is granted a Processing facility temporary equipment or 3176 <br />operational failure waiver, in accordance with Section 5.13 of the Agreement, the 3177 <br />Contractor shall include the following documents and information: 3178 <br />i. The number of days the Processing Facility temporary equipment waiver or 3179 <br />operation failure waiver was in effect; 3180 <br />ii. Copies of any notifications sent to the City pursuant to Sections 5.13.1, 3181 <br />5.13.2, and 5.13.3 of the Agreement; 3182
