EXHIBIT A
BID #OSD 2021-604
sanitary evaluation. The evaluations will locate and identify sanitary deficiencies and compliance with present
American Water Works Association (AWWA), Occupational Safety and Health Administration (OSHA), and
other applicable California standards.
The underwater diving portion of the tank inspection will implement AWWA, OSHA, and other applicable
California standards. Prior to entering the water, the diver will be wearing a volcanized rubber dry suit that is
in proper working condition. All materials, personnel and equipment coming in contact with potable water
inside the tank or reservoir will be thoroughly disinfected in a 2ooppm solution of chlorine and water. Our
dive crew will have a three-way hard wired communication at all times. Cathodic protection reference probe
inside tanks will be removed and replaced as applicable.
4.3. TANK CLEANING
Our team will remove dirt, debris, and sediment accumulated on the facility floors, walls, and columns without
creating any visible turbidity. Cleaning will combine adequate brushing of surfaces in addition to removing
material from the reservoirs by water suction. If piles of sand, debris, failed coating, rust scale or rocks are
discovered in the reservoir, it may not be possible to use Catalyst's standard vacuuming practices. This will
be considered a changed condition and a City representative will be contacted before Catalyst proceeds with
completing the cleaning.
Catalyst Diving will provide the City with real time HD videos illustrating the cleaning of each reservoir and a
detailed narration of what was seen by the diver during the inspection and cleaning.
4.4. SEDIMENT DISPOSAL
Catalyst Diving will collect a sediment sample for analysis by an independent laboratory and will submit
the certified lab analysis to the City prior to scheduling the removal of sediment. Catalyst Diving will legally
dispose of all tank and reservoir sediment and inform the City in writing that all sediment has been legally
disposed of. The removal pricing is based on all material removed from the tanks being classified as non-
hazardous or non-regulated substances allowing disposal at the local landfill. Should sediment be determined
to be hazardous waste or a regulated substance, an amendment to this Agreement will be approved by a City
representative prior to removal of solids.
4.5. DELIVERABLES
Catalyst Diving will provide the City two (2) hard copies and one electronic copy ofthe written inspection report
for each reservoir. Each report will contain color photographs of the tank's internal and external components.
The photographs will provide additional documentation of the tank's condition. Included in the report will be
a schematic drawing of the tank and reservoir that has a numerical system for the sections of the reservoir and
documentation of amounts and types of material found. In addition, the report will provide recommended
repairs based on the inspection results and will be grouped by priority as identified in the Proposal.
Our team will provide the City an inspection video with live narration to provide reasonable documentation of
the condition of the potable water storage facility interior before and after completion of work.
CATALYST 7
ti DIVING
