No operational changes that affect trip-scheduling, <br /> hours of operation, response time or any other characteristic of the PPS can be made by the <br /> CONTRACTOR without the prior written approval of the CITY. <br /> F. Reporting and Invoicing Requirements <br /> The CONTRACTOR must maintain a computerized reservation program to track trips and <br /> related operating statistics. The CITY receives funds from the Alameda County Transportation <br /> Commission(ALAMEDA CTC) and is subject to detailed reporting requirements. ALAMEDA <br /> CTC's reporting requirements may periodically change; the CITY will inform the <br /> CONTRACTOR of any changes ALAMEDA CTC makes to the reporting requirements and <br /> subsequent data that would be needed from the CONTRACTOR. See EXHIBIT B for Service <br /> Definitions of Operating Terms. <br /> The CONTRACTOR must become familiar with all applicable reporting requirements. The <br /> CONTRACTOR shall be liable for any funding penalties or late fees imposed by funding <br /> sources that are due to the CONTRACTOR's failure to comply with reporting requirements. <br /> CONTRACTOR shall submit an invoice and service reports electronically to the CITY within <br /> ten(10) calendar days of the end of each service month. <br /> G. Required Submittals <br /> CONTRACTOR is required to submit the following to the CITY each month: <br /> 1) Service Invoice- CONTRACTOR will provide the following on the monthly invoice: <br /> • Number of service hours billed for each service component,revenue hour reimbursement <br /> rate and service cost sub-total for door-to-door trips. <br /> • Total amount of passenger fares collected from both locations (CONTRACTOR office <br /> and CITY); this amount will be retained by the CONTRACTOR <br /> • Total miles driven per vehicle. <br /> 2) Data Summary Report by Service Day- CONTRACTOR will include the following data for <br /> each service day: <br /> • Number of service vehicles in use <br /> • Door-to-door Service: Number of one-way passenger(no attendant or companion trips) <br /> trips performed, cancelations, late cancelations, no-shows and missed trips, service <br /> hours,productivity and on-time performance. <br /> • Fares collected <br /> • Number of personal injury occurrences <br /> • Number of vehicle/property damage occurrences <br /> 3) Trip Detail Report for Door-to-Door Transportation Service-CONTRACTOR will include <br /> the following for each program participant's ride request: <br /> • Rider's last and first name <br /> • Assigned vehicle route number for the ride reservation <br /> 11 I Page <br />
