Attachment 2 <br /> • Date of the requested trip <br /> • Pick-up address <br /> • Pick-up City <br /> • Drop-off address <br /> • Drop-off city <br /> • Ride status (i.e. performed trip, canceled, no-showed, canceled late, or missed trip) <br /> • Whether a personal care attendant accompanied a rider on a performed trip <br /> • Whether a companion accompanied a rider on a performed trip <br /> • Whether the performed trip was for a rider who used the lift <br /> • Whether the performed trip was a demand response or standing order ride <br /> 4) On-Time Performance Report for Door-to-Door Transportation Service - CONTRACTOR <br /> will include the following for each program participant's ride request: <br /> • Rider's last and first name <br /> • Assigned vehicle route number for the ride reservation <br /> • Date of the requested trip <br /> • Whether the ride was performed, canceled, no-showed, canceled late, or a missed trip <br /> • Scheduled pick-up time and appointment time, if provided by rider <br /> • Actual pick-up time and drop-off time <br /> • Whether a late performed or no-showed ride was 1-20 minutes late, 21-59 minutes late <br /> or 60 or more minutes late <br /> • Ride time for each performed trip <br /> Note: Reports 3 and 4 above can be combined into one report if the CONTRACTOR wishes to <br /> do so. <br /> CONTRACTOR shall also provide copies of the driver's manifest and dispatch log(via email) <br /> when requested by the CITY.These manifests and logs must be provided within one(1)business <br /> day of the CITY's request. Dispatch logs and driver manifests should include documentation on <br /> passenger"no-shows", trip cancellations, fare evasions, trip denials, missed trips and any other <br /> relevant data that details what may have happened with a participant's trip. <br /> VIII. LICENSING, PERMITS AND CERTIFICATIONS <br /> The CONTRACTOR will obtain all required state and local permits and ensure that all drivers <br /> are properly licensed for the service they are providing. CONTRACTOR must also hold all <br /> applicable state and local business licenses.The CONTRACTOR will ensure that all equipment, <br /> facilities,personnel and other program elements operate in conformance with applicable federal, <br /> state and local laws. <br /> IX. FACILITIES,EQUIPMENT AND SERVICES <br /> The CONTRACTOR will furnish all facilities, equipment, vehicle maintenance, and services <br /> required in the operation and management of the PPS. The CONTRACTOR must supply an <br /> adequate vehicle storage facility. This facility should be located within reasonable distance of <br /> 12IPage <br />
