backfilled with 2 sack slurry and re-paved. The additional work is being <br /> completed under time and material. The estimated cost increase is <br /> $12,000. <br /> b. South Isolation Valve Tie In. A new isolation valve for the southside of the <br /> bridge was scheduled to be installed at the flex tend installation to support <br /> the shutdown of the water line. Due to a conflict with a light pole and <br /> existing utilizes a new location 35' south of the work had to be selected. <br /> When excavating a streetlight power conduit had to be relocated. <br /> Additional work for a new excavation, fittings and electrical work is <br /> estimated at$13,500. <br /> c. Hazardous Pipe Removal. At the new isolation valve tie-in locations, the <br /> pipe was found to be ACP instead of the DIP found at the bridge. A <br /> specialty hazardous material contractor was brought in to perform the <br /> cutting and removal of the ACP pipe. The cost to complete the work is <br /> estimated at $9,500. <br /> d. North Flex Tend Installation. At the north flex tend installation the existing <br /> pipe coming through the bridge back wall has sagged down several <br /> inches exceeding the pipe manufactures recommendations at the joint. <br /> The pipe will be removed, and a new piece of pipe will be installed back <br /> the existing bell in the bridge at Time and Material. Neoprene will be <br /> added around the pipe and the backfill grouted as the existing is a rough <br /> jack hammered opening. The estimated cost for work is $10,000. <br /> e. Restoration. Due to the additional tie-in locations and exploratory digging <br /> to find the pipes additional curb and gutter and planting is required. The <br /> estimated cost for restoration is $5,000. <br /> 10. PCO#10—$5,000 For Bridge 33C-0099. Demo concrete and custom paving <br /> seat in SW corner around shallow gas line coming thru the bridge. <br /> 11. PCO#11 - $3,000 For Bridge 33C-0099. Remove old concrete kicker at south <br /> flex-tend that interferes with the pipe installation. (Non-participating) <br /> 12. PCO#12 - $6,500 For Bridge 33C-0099 Replace one K-rail crash attenuator. <br /> (Two damaged to-date) <br /> 13. PCO#13 - $30,000 For Bridge 33C-0099. Install a leveling course to return the <br /> bridge deck to grade. The contract plans identified removal and installation of 2" <br /> of asphalt pavement. When removing the pavement, it was found that there was <br /> a grade break in the deck and a wedge of asphalt about 11"thick was used to <br /> create 2% cross slope in the top pavement surface. The work involves placement <br /> of approximately 100 tons of leveling. <br /> 14. PCO#14 - $42,500 For bridge 33C-0099. The City of Pleasanton is overlaying <br /> Santa Rita Road as part of annual resurfacing program. At the time of bid it was <br /> anticipated the work at the bridge would be completed prior to the City's paving <br /> project. The paving was pushed to the end of the schedule but due to <br /> complications with the water line and adding the additional phase of construction <br /> the work on the bridge is not completed. The limits of paving needs to be <br /> extended approximately 65' beyond the bridge approach slabs to not interfere <br /> with work on the bridge. This will add approximately 170 tons of additional paving <br /> 2 <br />
