CCO#1 - $54,435.82 Additional phase of construction southbound Santa <br /> Rita/Arroyo Mocho. The project design identified two phases of construction for <br /> the southbound direction, but the roadway narrows prior to the bridge and a third <br /> phase had to added to keep enough open road width to maintain two lanes of <br /> traffic. <br /> 2. CCO#2 - $7,748.66 Removal of concrete base in the NW corner of Santa <br /> Rita/Arroyo Mocho. <br /> 3. CCO#3 - $4,256.20 Extra Concrete flat work in the SW corner of Santa <br /> Rita/Arroyo Mocho. <br /> 4. CCO#4 — ($10,000) Southeast corner credit for reduction in approach slab work <br /> at Santa Rita/Arroyo Mocho. <br /> 5. CCO#5 — ($3,162) Credit for salvaging existing streetlight and electrolier at Santa <br /> Rita/Arroyo Mocho. (Non-participating work.) <br /> Proposed Change Order <br /> 6. PCO #6 - $68,000 For Bridges No 33C-0461 (West Las Positas Boulevard at G- <br /> 3) and 33C-0462 (Owens Drive at Chabot Channel) additional epoxy crack <br /> injection associated with contract line item 9. The engineer estimated a total of <br /> 853 linear feet of treatment, but field mark outs identified 2,213 linear of repairs. <br /> The design engineer field verified the conditions and recommended the work to <br /> be completed. The additional quantity is attributed to an error in design not <br /> appropriately accounting for how payment on shared walls were to take place; <br /> additional cracking from time of design to construction; and extension of cracking <br /> further than anticipated after cleaning. See attached letter from Biggs Cardosa <br /> and Associates. The work was agreed to be completed at contract unit price for a <br /> total increase in project costs of$68,000. <br /> 7. PCO #7- $8,222 For Bridge 33C-0099 Additional curb and gutter and sidewalk <br /> and driveway work in SE corner of Santa Rita/Arroyo Mocho to remove ponding <br /> water due to grade changes at the bridge. <br /> 8. PCO #8 -$12,500 For bridge 33C-0099 Extend temporary K-Rail and crash <br /> cushions at Santa Rita/Arroyo Mocho to improve safety after three collisions into <br /> the K-rail. <br /> 9. PCO #9 - $50,000. For bridge 33C0099 (Santa Rita Road at Arroyo Mocho) to <br /> address additional work associated with line item 37 Water Main Flexible <br /> Expansion Joints. (Non-participating) <br /> a. North Isolation Valve Tie In. The water line was not located where record <br /> drawing identified. An additional location within the roadway had to be <br /> excavated. The pipeline was 4' deeper than anticipated requiring a shore <br /> box and additional excavation and backfill.
