Box 520 <br /> Pleasanton, CA 94566 <br /> Re: Community Development Block Grant(CDBG)—COVID-19 Emergency Funding <br /> Dear Becky: <br /> Thank you for the information regarding additional CDBG funds that are being awarded to the City of <br /> Pleasanton for the continued response to the COVID pandemic. As we discussed,the intent of these <br /> funds is to assist communities in providing COVID-related services for low income residents. We were <br /> most grateful to have been awarded CDBG funds earlier this year in this effort, and we look forward to <br /> support for our continued work, as we battle the ongoing COVID pandemic in our community. Per your <br /> request,this is to describe our proposal to continue this important work with an additional award of <br /> $150,000,which will be used between January 1, 2021 and December 31, 2021: <br /> Organizational description <br /> Axis Community Health (Axis) is a nonprofit community health center that has been serving Eastern <br /> Alameda county since 1972. Axis's mission is to provide medical, behavioral health and supportive <br /> services for all members of the community, regardless of income or any other factor. Axis is the only <br /> provider of primary care medical services for Eastern Alameda County's low income population. More <br /> than 94%of Axis's patients have a family income that is less than 200%of the Federal Poverty Level, <br /> which includes 74% who have a family income that is below 100%of the FPL. Axis's services include <br /> medical care, mental health counseling, dental care, WIC nutrition services, and a wide range of support <br /> services that ensure ongoing access to care. Axis has a staff of 175 and provides services at five sites <br /> that are located throughout the Tri-Valley, including three in Pleasanton. Axis is a designated Federally <br /> Qualified Health Center, which means that we fulfill all requirements of the US Department of Health <br /> and Human Services/ Bureau of Primary Care regulations that are specific to the provision of medical <br /> care for low income residents. In 2019, Axis served more than 14,000 Tri-Valley residents and provided <br /> more than 90,000 medical/mental health/dental visits. <br /> Project Proposal <br /> Under the Housing and Urban Development guidelines for the funding,Axis proposes the continuation <br /> of services that prevent the spread of COVID-19 and respond to the health care needs of our <br /> community's most vulnerable population.
