and telehealth medical services to assess patients for symptoms of COVID-19 or other infectious <br /> diseases, (2) diagnostic, treatment and follow-up care for patients who are at-risk for or diagnosed with <br /> COVID-19 or other infectious diseases, (3) coordinating related ancillary services, to include testing, <br /> contact tracing, pharmaceuticals and home diagnostic equipment as appropriate, and (4) patient, family <br /> and community education regarding infectious disease prevention. In order to conduct these services, <br /> When disease prevention modalities become available, Axis will develop and implement strategies to <br /> provide these enhanced services for patients as well. <br /> Axis will provide these services at its three clinic sites,which are located at 3311 Pacific Avenue in <br /> Livermore,4361 Railroad Avenue in Pleasanton,and 5925 W. Las Positas Blvd., also in Pleasanton. This <br /> project will serve approximately 2,400 Pleasanton residents who are patients at Axis; an estimated 1,775 <br /> (74%)of these patients will have a family income that is less than 100%of the Federal Poverty Level <br /> (FPL) and 480 (20%) will have a family income that is less than 200%of FPL. <br /> Need for Funds <br /> Since March 2020, Axis has been engaged in a multitude of efforts to prepare, prevent and respond to <br /> the COVID pandemic in the Tri-Valley. During this time, we have converted 90%of our medical and <br /> behavioral health visits to a telehealth format to ensure the continuation of essential services for the <br /> Tri-Valley's most vulnerable residents. We have developed "clean clinics" at our three clinic sites,which <br /> allow us to provide in-person visits for patients who have conditions that cannot be managed in a virtual <br /> health environment. We have begun a COVID testing service,we provide outpatient care for patients <br /> who have contracted COVID, and we have implemented home monitoring strategies for patients who <br /> are acutely ill, which include monitoring blood pressure, temperature and oxygen levels remotely. We <br /> also continue to coordinate our efforts with the Alameda County Department of Public Health, to ensure <br /> that contact tracing is completed and that testing services are integrated with the County data-reporting <br /> platform. <br /> The cost of these efforts have been immense. We have also suffered revenue losses for the services <br /> that had to be discontinued or reduced in order to prevent the spread of disease (dental care, <br /> acupuncture and chiropractic services have been significantly reduced). We were fortunate to have <br /> received support for some of these efforts from the Cities of Pleasanton and Livermore. Additional <br /> support was received from the US Department of Health Resources Administration, as part of the <br /> nationwide pandemic funding strategy. Alameda County has supported some of our COVID testing <br /> services as well. However, despite this support, we continue to record operational deficits, due to the <br /> increased demand for services, coupled with the loss of revenues for services that required reductions. <br /> Nearly all of these additional funds that were received require full expenditure by the end of 2020, so <br /> looking forward, we have minimal support to continue our efforts. <br /> CDBG support from the City of Pleasanton is critical to our continuing efforts on this front, as the funds <br /> we received from federal and county sources will be fully expended in the coming months. We further <br /> anticipate that we will play a significant role in the community once a vaccine is ready for administration <br /> to the public.
