Finding Wellness Program — nutrition, exercise, alcohol, medication and drug safety <br /> education <br /> Health Screenings — provided at a variety of senior sites in Eastern and Central <br /> Alameda County and includes; foot care, blood pressure screening, diabetes screening, <br /> hemoccult testing and urine testing <br /> In-Home Caregiver Registry — provides a vetted list of workers for housework, home <br /> care and personal care <br /> In-home Counseling — provides one-on-one counseling and emotional support for Tri- <br /> Valley Seniors <br /> Nutrition Education and Fitness — offers individual and group nutrition education, <br /> group fitness, fall prevention, Tai Chi, and in-home exercise <br /> Senior Transportation Program —volunteer drivers provide eligible seniors with door- <br /> through-door transportation to medical appointments <br /> Stepping out and Reconnecting (SOAR) — combines recreational, therapeutic and <br /> educational activities to inspire seniors to re-engage in their community and holistically <br /> improve overall well-being <br /> The recommended Agreement for Services is for one year with the option of four <br /> additional one-year extensions. Any extension will be based on an assessment of the <br /> quality of service and City Council approval.
