The City of Pleasanton is seeking a provider(s) to address Health Care and <br />Behavioral Health. As identified in the City of Pleasanton Human Services <br />Needs Assessment Updated Strategic Plan 2018-2022, the successful <br />recipient(s) must display a creative approach to addressing identified needs <br />including: <br />o Wrap around services <br />o Coordinated care between service providers <br />o Crisis response <br />c Prevention <br />o Subsidized dental care for low income residents <br />o Populations of focus - domestic violence victims, elderly, homeless and <br />youth <br />In addition to the bulleted needs listed previously the following has been identified <br />as essential factors to be successful with this RFQ: <br />1. Assessments based on mental health <br />a. Assess clients regarding individual needs, i.e. physical and mental <br />health, housing, food, and employment. <br />2. Crisis intervention services <br />a. Assisting with mental health needs <br />b. Life skills classes <br />c. Crisis response <br />d. Meet clients where they are <br />e. Ability to arrange for the transport of clients <br />f. Provider(s) must work closely and share confidential information <br />with Pleasanton Police Department staff <br />3. Who we want to serve with funds from this RFQ (extremely low income (<30/ <br />AMI) and very low income (<30%-50% AMI) <br />a. Clients must be a resident of Pleasanton <br />b. Clients must be underserved, elderly, youth or homeless. <br />c. Clients who need access to clinical assistance / mobile crisis <br />The City of Pleasanton is looking for creative solutions and proposals. Successful <br />recipient(s) will provide a creative approach to the health care and behavioral health <br />most critical human service need and do so within the constraints of available funding. <br />III. Funding <br />Funds available for this RFQ are $35,000 per fiscal year, not -to -exceed <br />$175,000 for a total estimated contract term of five years. <br />
