Maintain communication with the registrant and his or her <br />family or caregiver during the assessment and planning stages. <br />Completed. <br />Prepare staff to implement the plan. <br />Completed. <br />Develop easy to use evaluation forms. <br />in -progress. <br />In June and November of 2017, employees from the Library and Recreation Department <br />and City Attorney's Office participated in inclusion training provided by the WT Group. <br />Trainings included the eight -step inclusion process, defining reasonable accommodations <br />and trends, and group discussion on scenarios and problem solving. In January of 2018, <br />an Ad -Hoc Inclusion Committee was formed, comprised of Library and Recreation and City <br />Attorney employees. <br />The ad-hoc committee has: <br />• Identified an Inclusion Coordinator from within the existing department <br />• Addressed the eight -step inclusion process as referenced above <br />• Modified current publications <br />• Developed an in -take form <br />• Completed an accommodation request process <br />• Finalized a Consent and Direction to Staff for the Self -Administration of Medicines <br />Plan and Form <br />• Launched an inclusion page for the City website. <br />Section 2 of the WT Group Final Report addressed some specific department processes <br />and materials.
