Currently, the Senior Support Program of the Tri-Valley is open Monday through <br /> Thursday at the Pleasanton Senior Center. Their programs and services include: <br /> Friendly Visiting, Case Management, In-home Caregiver Registry, Family Caregiver <br /> Support, Alcohol and Drug Abuse Prevention, Education and Outreach, Exercise and <br /> Nutrition Classes, Health Screenings, Information and Referral Services. <br /> The proposed Service Contract with the Senior Support Program of the Tri-Valley will <br /> also address and meet the following needs as prioritized in the 2014 Human Services <br /> Needs Assessment - Strategic Plan: Disability Services and Access, Food and Nutrition, <br /> Health Care and Senior Services. It also addresses the increasing need of the growing <br /> senior population in Pleasanton by adding one (1) additional day of programs and <br /> services with an on-site Case Manager, as well as providing additional Foot Care <br /> Services. The scope of services also includes the following: <br /> Friendly Visiting is for the individual who — because of age, chronic illness, or disability <br /> — cannot get out and make friends. A Friendly Visitor is a link with the community and <br /> an advocate. This service decreases social isolation and loneliness while enabling the <br /> Friendly Visitor to identify needs and assess the senior's environment. <br /> Case Management includes comprehensive assessments, care planning, <br /> authorizing/arranging services, crisis intervention services, coordinating/monitoring, <br /> periodic follow-up, reassessment and/or revision of care plans. Case Management is <br /> the critical link between seniors in need and assistance available in the community. <br /> In-home Caregiver Registry provides names of pre-screened workers for home care <br /> and personal care. The Registry Coordinator meets with clients to assess their needs <br /> and provided information of appropriate caregivers that best match the senior's needs. <br /> Family Caregiver Support is provided through the Caregiver Support Group and <br /> Movin' On Group. These groups are for individuals to find support to receive <br /> reassurance, learn available resources, gain coping skills, get assistance, and connect <br /> with other caregivers and staff. <br /> Alcohol and Drug Abuse Prevention is provided through a GROW (Growth, <br /> Recovery, and Other Wellness) Group and the Finding Wellness - Alcohol/Drug <br /> Education and Prevention program. <br /> Education and Outreach is provided to Pleasanton Seniors and their families on a <br /> variety of topics and needs. <br /> Exercise and Nutrition Classes are offered weekly. A variety of exercise classes are <br /> offered such as: Group Exercise classes, Fall Prevention Exercise classes and <br /> customized exercise classes for a senior in their home. Nutrition classes include Lighten <br /> Up Talks and Fresh Bites Cooking Demonstrations. <br /> Page 3 of 4 <br />
