Health Screenings provide general screenings to determine the need for more in-depth
medical evaluation and referrals. Services include: blood pressure measurement,
colon/rectal cancer screening, blood sugar/diabetes screening, lung assessment, urine
dipstick screening, hemoccult testing, alcohol consumption, and smoking cessation.
Information and Referral Services are provided daily to Pleasanton Seniors and their
families as they call or walk-in to receive information about available services.
Foot Care is provided by health screening nurses; checking for edema, caring for
toenails and checking for sores, cuts and ulcers on the feet. Nurses also discuss other
social or health related concerns, providing support, information and resources to
seniors. Currently there is a three (3) month waiting list for a foot care appointment.
Increased funding in this area will help to meet the current need.
The recommended Service Agreement is for one (1) year with the possible option of
four (4) additional one year extensions. Any extension will be based on an assessment
of the quality of service. As in previous years, staff will continue to monitor the oversight
of the Service Agreement and the Senior Support of the Tri-Valley will provide monthly
reports with service metrics, as well as present their annual report to the Human
Services Commission.
Submitted by: Fiscal Review: Approved by:
/AN_
Susan Andrade-Wax Tina Olson Nelson Fialho
Director of Community Services Director of Finance City Manager
Attachment:
1.
