11. Annual Fitness Evaluation following medical clearance (same as baseline) <br /> 12. Individual Health Risk Appraisal (Continued annually after baseline) <br /> 13. Follow-Up or Referral to Health Care Practitioner <br /> The Wellness-Fitness Initiative recognizes the importance of consultation and/or <br /> referral to outside health care providers and/or specialists. Aspects of the follow- <br /> up and referral program include: <br /> A. Abnormal findings on the annual physical must be addressed by follow-up <br /> or referral. <br /> B. Revaccination or intervention following exposures must be managed by <br /> follow-up or referral. <br /> C. Return to work determinations that require clearance by the LPFD <br /> Physician or other Provider following a consult with an outside MD. <br /> D. Follow-up findings from annual examinations must be reviewed by LPFD <br /> Physician. <br /> Task 3 - Provide occasional specialty services for LPFD suppression personnel (involved <br /> with Hazard Materials or Urban Search and Rescue: <br /> 1. Draw (in addition to the blood chemistry for an annual exam) blood chemistry <br /> and urine screening for some of the following heavy metals: Arsenic, <br /> Cadmium, Lead, Bismuth, Beryllium, Chromium, and Mercury or any other <br /> necessary blood exam. <br /> Task 4 - Workers Compensation Consultation <br /> LPFD currently has an established Workers Compensation Provider. The LPFD Physician <br /> would serve as an adjunct to confer and consult with City's contracted occupational health <br /> provider or Firefighter's private medical doctor regarding: <br /> 1. Diagnosis of injury or illness <br /> 2. Prescribed rehabilitation program from injury or illness <br /> 3. Pertinent past medical history <br /> Task 5 - Maintain information collected in a confidential manner. Information collected will <br /> assist physician with identifying the effectiveness of the Medical & Fitness Program in <br /> improving an individual's physical fitness. <br /> Page I 1 of 16 <br />
