Follow-up findings from annual examinations must be reviewed by the <br /> LPFD physician. <br /> Task 2 - Provide annual medical and fitness evaluations for LPFD uniformed employees: <br /> Annual: <br /> 1. Medical History Questionnaire (update annually) <br /> 2. Hands On Physical Exam (same as baseline) Annual physical evaluation will <br /> establish an ongoing record for current members ensuring ability to perform <br /> essential job duties while detecting changes in an individual's health. <br /> Recommendations and education in disease prevention and health promotion will <br /> be provided to each department member specific to identified needs. In addition, <br /> information will be provided to each member regarding any occupational hazards, <br /> which may impact their current health. <br /> 3. Body Composition (same as baseline) <br /> 4. Blood Analysis (same as baseline) <br /> 5. Urinalysis (same as baseline) <br /> 6. Vision& Hearing Tests (same as baseline) <br /> 7. Pulmonary (same as baseline w/o chest x-ray) <br /> 8. EKG (resting) <br /> 9. Cancer Screening Elements (same as baseline) <br /> *Components of this exam can be deferred when employee uses his/her own <br /> primary care physician to perform exam.
