Flexibility: sit& reach <br /> 4. Feedback and Follow-Up <br /> A. Individual Health Risk Appraisal (conducted by Physician) <br /> 1. Written feedback to uniformed personnel concerning risks and health status <br /> is required following an exam. <br /> 2. Develop a plan tailored for each member reducing risk factors while <br /> assisting LPFD members to improve overall health and well being. <br /> B. Follow-Up or Referral to Health Care Practitioner (conducted by Physician) The <br /> Wellness-Fitness Initiative recognizes the importance of consultation and/or <br /> referral to outside health care providers and/or specialists.
