14.6 Military Leave <br /> Military leave shall be granted in accordance with the provisions of the law. All <br /> employees entitled to military leave shall give the Appointing Authority an <br /> opportunity within the limits of military regulations to determine when such leave <br /> shall be taken. <br /> 14.7 Failure to Return from Leave <br /> Failure on the part of the employee on leave to report to duty promptly upon its <br /> expiration or within a reasonable time after its expiration shall be cause for <br /> discipline. <br /> Section 15. Health and Welfare <br /> 15.1 Hospital and Medical Care - Active Employees <br /> The City shall contribute an amount necessary to provide hospital and medical <br /> care benefits for the individual employees and eligible dependents; provided, <br /> however, in no event shall the City be obligated to contribute a monthly amount - <br /> in excess of the monthly premium for the Kaiser S -1 Health Plan full family <br /> coverage. <br /> Effective November 1, 1996 the City's monthly premium contribution for the <br /> Kaiser S -1 medical care benefits shall include the premium for the Kaiser durable <br /> medical benefit. <br /> Effective June 1, 1998, the City shall modify the Kaiser S -1 plan to include the <br /> Kaiser "CH3" chiropractic plan for the employee and eligible dependents. The <br /> cost of said plan shall be borne by the City. <br /> For the purpose of providing aforesaid health care coverage to eligible regular <br /> part-time employees including job share employees who average between twenty <br /> (20) and twenty -nine (29) hours per week, the City shall contribute an amount not <br /> to exceed fifty percent (50 %) of the City's monthly contribution established for <br /> full time employees as set forth in the paragraph above. For regular part -time <br /> employees including job share employees who average between thirty (30) and <br /> thirty-nine (39) hours per week, the City shall contribute an amount not to exceed <br /> seventy-five percent (75 %) of the City's monthly contribution above. <br /> 15.2 Health and Medical Care - Retirees <br /> Effective April 30, 1990, for all employees who retire for service after April 30, <br /> 1990 the City shall pay for each year of service, four percent (4 %) of the monthly <br /> premium for employee and one dependent of the City's current Kaiser S -1 Health <br /> Plan coverage.
