14.3 <br /> <br />Leaves Resulting from Subpoena <br /> <br />14.4 <br /> <br />Leave of absence with pay shall be granted to a person <br />while going to and from court and answering a subpoena as <br />a witness. The employee shall be paid the difference <br />between his/her full salary and any payment received <br />by him/her, except travel pay, for such duty. <br /> <br />Military Leave <br /> <br /> ~litary leave shall be granted in accordance with the <br /> provisions of law. All employees entitled to military <br /> leave shall give the Appointing Authority an opportunity <br /> within the limits of military regulations .to determine <br /> when such leave shall be taken <br /> <br />Section 15. Health and Welfare Coverage <br /> <br />15.! Hospital and Medical <br /> <br />Effective April 1, 1981, 'the City shall com~ribute an <br />amount necessary to provide hospital and medical care <br />bene~its..under the present Blu~ cross Plan 'or Kaiser S <br />Plm~-for the individual employee and a portion of the <br />amount needed to provide coverage for eligible dependents; <br />provided, however, that in no event shall the City be <br />obligated. to contribute an amount in excess of One <br />Hundred Dollars ($100.00) per month per employee to <br />provide the aforesaid coverage. Effective January 1., <br />1982, the City's maximum monthly contribution shall <br />increa'se to an amount not to exceed One Hundred Ten <br />Dollars {$110.00) per month. <br /> <br />15.2 <br /> <br />Effective January 1; 1982, the City agrees to amend <br />its Kaiser S health plan coverage for the purposes of <br />adding the vision care and prescription drug options <br />if the Union desires s~id options. "'- <br /> <br />Dental <br /> <br />Effective kpril 1, 1981, the City shall contri~,ute an <br />amount necessary to provide dental care benefits under <br />the City's present Bankers Life Nebraska Plan for the <br />individual employee and eligible dependehts; provided, <br />however, that in no event shall the ~ity be obligated <br />to contribute an amount in excess of ~venty-eight <br />Dollars and eighty cents' ($28.80) per month per employee <br />to provide the aforesaid coverage. <br /> <br />15 . 3 <br /> <br />Life, Accidental Death & Dismemberment and Long Term+ <br />Disability <br /> <br />The City shall contribute an amountn~cessary to provide <br />life~ accidental death and dismemberment, and long term <br />disability insurance under the City's present Standard <br />Insurance Company Group Policy for the individual employee; <br />provided, however, that in no event shall the City be <br />obligated to contribute an amount in excess of One Dollar <br />and seven cents {$1.07) per One Hundred Dollars ($100) of <br />m~nthly salary of the fir~t $2250 of monthly salary not <br />to exceed a total contribution of Twenty-four Dollars and <br />eight cents ($24.08) per month per pmployee. <br /> <br />-ll- <br /> <br /> <br />
