14.3 Leaves Resulting from Subpoena 14.4 Leave of absence with pay shall be granted to a person while going to and from court and answering a subpoena as a witness. The employee shall be paid the difference between his/her full salary and any payment received by him/her, except travel pay, for such duty. Military Leave ~litary leave shall be granted in accordance with the provisions of law. All employees entitled to military leave shall give the Appointing Authority an opportunity within the limits of military regulations .to determine when such leave shall be taken Section 15. Health and Welfare Coverage 15.! Hospital and Medical Effective April 1, 1981, 'the City shall com~ribute an amount necessary to provide hospital and medical care bene~its..under the present Blu~ cross Plan 'or Kaiser S Plm~-for the individual employee and a portion of the amount needed to provide coverage for eligible dependents; provided, however, that in no event shall the City be obligated. to contribute an amount in excess of One Hundred Dollars ($100.00) per month per employee to provide the aforesaid coverage. Effective January 1., 1982, the City's maximum monthly contribution shall increa'se to an amount not to exceed One Hundred Ten Dollars {$110.00) per month. 15.2 Effective January 1; 1982, the City agrees to amend its Kaiser S health plan coverage for the purposes of adding the vision care and prescription drug options if the Union desires s~id options. "'- Dental Effective kpril 1, 1981, the City shall contri~,ute an amount necessary to provide dental care benefits under the City's present Bankers Life Nebraska Plan for the individual employee and eligible dependehts; provided, however, that in no event shall the ~ity be obligated to contribute an amount in excess of ~venty-eight Dollars and eighty cents' ($28.80) per month per employee to provide the aforesaid coverage. 15 . 3 Life, Accidental Death & Dismemberment and Long Term+ Disability The City shall contribute an amountn~cessary to provide life~ accidental death and dismemberment, and long term disability insurance under the City's present Standard Insurance Company Group Policy for the individual employee; provided, however, that in no event shall the City be obligated to contribute an amount in excess of One Dollar and seven cents {$1.07) per One Hundred Dollars ($100) of m~nthly salary of the fir~t $2250 of monthly salary not to exceed a total contribution of Twenty-four Dollars and eight cents ($24.08) per month per pmployee. -ll-
